中药自拟方对老年腹股沟疝术后肠道功能恢复疗效观察

【摘要】 目的 探讨消肿通便自拟方在老年腹股沟疝患者全身麻醉术后肠道功能恢复中的作用。方法 收集2020年1月至2022年12月291例接受全身麻醉腹股沟疝无张力修补术的老年患者的临床资料，按术后24 h给予消肿通便方分为中药治疗组（161例）与对照组（130例）。比较两组患者术后首次排便时间、住院时间、并发症等。应用Cox回归模型分析影响术后首次排便时间的因素。结果 中药治疗组患者首次排便时间（49.2±15.3）h，对照组（64.7±27.6）h，差异有统计学意义（P<0.001）。中药治疗组术后住院时间（3.8±1.3）d，对照组（4.6±2.4）d，差异有统计学意义（P=0.002）。多因素分析显示，术后是否应用消肿通便方（HR=2.421,95%CI:1.875～3.126,P<0.001）及手术时间（HR=0.997,95%CI:0.994～1.000,P=0.042）是影响术后首次排便时间的独立因素。结论 消肿通便方能促进老年腹股沟疝患者全身麻醉术后肠道功能恢复，加速患者术后康复。更多还原

【Abstract】 Objective To investigate the effect of self-designed Xiaozhong Tongbian Decoction on intestinal function recovery in elderly patients with inguinal hernia after general anesthesia. Methods The clinical data of 291 elderly patients who underwent tension-free inguinal hernia repair under general anesthesia from January2020 to December2022 were collected. According to whether Xiaozhong Tongbian Decoction was given 24 h after operation, the patients were divided into traditional Chinese medicine treatment group(161 cases) and control group(130 cases). The first postoperative defecation time, hospitalization time and complications were compared between the two groups. Cox regression model was used to analyze the factors affecting the first postoperative defecation time. Results The first defecation time was(49.2±15.3)h in the traditional Chinese medicine treatment group and(64.7±27.6)h in the control group, with statistically significant difference(P<0.001). The postoperative hospitalization time was(3.8±1.3)d in the traditional Chinese medicine treatment group and(4.6±2.4)d in the control group, with statistically significant difference(P=0.002). Multivariate analysis showed that whether Xiaozhong Tongbian Decoction was used after operation(HR=2.421,95% CI: 1.875～3.126,P<0.001) and operation time(HR=0.997,95% CI: 0.994～1.000,P=0.042) were independent factors affecting the first postoperative defecation time. Conclusion Xiaozhong Tongbian Decoction can promote the recovery of intestinal function and accelerate the postoperative rehabilitation of elderly patients with inguinal hernia after general anesthesia.更多还原
