【摘要】 目的 探讨糖尿病周围神经病变（DPN）患者抑郁情况，分析影响因素及其与神经性疼痛的关系。方法 选取184例DPN患者为研究对象，采用病人健康问卷-9评估DPN患者抑郁情况，按照DPN患者是否发生抑郁分为抑郁组及无抑郁组。统计两组患者临床资料及神经性疼痛情况，行单因素分析及多因素Logistic回归分析明确DPN患者抑郁发生影响因素。结果 184例DPN患者发生抑郁89例（48.37%），纳入抑郁组；未发生抑郁患者95例（51.63%），纳入无抑郁组。两组患者性别、文化程度、家庭月收入、合并视网膜病变、糖尿病病程、糖化血红蛋白（HbA1c）及神经性疼痛发生情况比较，差异均有统计学意义（P<0.05或0.01）。多因素Logistic回归分析结果显示，性别、家庭月收入、糖尿病病程、HbA1c及神经性疼痛是DPN患者发生抑郁的影响因素（P<0.01）。结论 DPN患者抑郁发生率较高，需重点关注女性、家庭月收入较低、糖尿病病程较长、血糖控制不佳及存在神经性疼痛的DPN患者。更多还原

【Abstract】 Objective To explore the depression status of patients with diabetic peripheral neuropathy(DPN),analyze its influencing factors and its relationship with neuropathic pain.Methods A total of184 DPN patients were selected as research objects,their depression status was evaluated using the Patient Health Questionnaire-9(PHQ-9),and they were divided into depression and non-depression group according to whether they developed depression.Clinical data and neuropathic pain conditions of both groups were counted,univariate and multivariate Logistic regression analysis carried out to clarify the factors influencing the occurrence of depression in DPN patients.Results Of 184 DPN patients,89(48.37%) developed depression and were included into depression group;95 patients didn’t develop depression and were included non-depression group.Intergroup differences in term of genders,educational levels,monthly family incomes,combined retinopathy,course of diabetes,glycated hemoglobin(HbA1c) and neuropathic pain were statistically significant(P<0.05 or 0.01).The results of multivariate Logistic regression analysis showed that gender,monthly family incomes,course of diabetes,HbA1c and neuropathic pain were the factors influencing the occurrence of depression in DPN patients(P<0.01).Conclusion The incidence of depression in DPN patients is higher,special attention should be paid to the DPN patients such as female,low monthly family income,longer course of diabetes,poor control of blood glucose,and neuropathic pain ones.更多还原
