【摘要】 目的 分析苯扎氯铵相关性接触性皮炎的临床特征、诊断要点及治疗策略。方法 回顾性分析2023年8—11月广东省中医院皮肤科收治的同一家庭3例苯扎氯铵相关性接触性皮炎患者的临床资料，结合病史、皮损特点、实验室检查、皮损组织病理表现、治疗及转归进行综合分析，并进行文献复习。结果 3例患者发病前均有使用含苯扎氯铵(70 g/L)衣物除菌液史。所有患者临床表现为躯干、四肢及皮肤褶皱部位对称性分布的红斑、丘疹、脓疱及鳞屑，其中2例患者皮损组织病理检查示角质层下脓疱。3例患者均诊断为苯扎氯铵相关性接触性皮炎。停用含苯扎氯铵产品并给予系统及局部糖皮质激素治疗后，所有患者皮损完全消退；其中1例再次接触原衣物后皮损复发，经同样治疗并彻底避免接触后未再复发。结论 苯扎氯铵是接触性皮炎的一种罕见过敏原，苯扎氯铵相关性接触性皮炎临床表现与角层下脓疱性皮病等疾病相似，易导致误诊，提示详细的病史询问和彻底避免接触过敏原是本病诊断及治疗的关键。更多还原

【Abstract】 Objective To analyze the clinical features, diagnostic principles, and treatment strategies of benzalkonium chloride-associated contact dermatitis. Methods A retrospective analysis was conducted on the clinical data of three patients from the same family with benzalkonium chloride-associated contact dermatitis, who were admitted to the Department of Dermatology at Guangdong Provincial Hospital of Traditional Chinese Medicine from August to November 2023.A comprehensive analysis was performed, integrating medical history, skin lesion characteristics, laboratory tests, histopathological findings, and treatment outcomes. Results All three patients had a history of using a laundry disinfectant containing benzalkonium chloride(70 g/L) prior to the onset of symptoms.The clinical presentation included symmetrically distributed erythema, papules, pustules, and scales on the trunk, limbs, and skin folds. Histopathological examination revealed subcorneal pustules in two patients.All three patients were diagnosed with benzalkonium chloride-associated contact dermatitis. After discontinuing the benzalkonium chloride-containing products and receiving systemic and topical glucocorticoid therapy, the skin lesions resolved completely in all patients. Notably, lesions recurred in one patient upon re-exposure to the original clothing, but resolved completely after repeated treatment and strict avoidance of the allergen. Conclusion Benzalkonium chloride is a rare allergen responsible for contact dermatitis. Its clinical manifestations can mimic other conditions such as subcorneal pustulosis, leading to potential misdiagnosis. A detailed medical history and a complete allergen avoidance are crucial for both diagnosis and management.更多还原
