【摘要】 目的:探讨输尿管硬镜联合阻石篮(N-trap)在处理输尿管超上段结石中的效果及手术经验。方法:回顾性分析2021年6月—2024年8月于北京大学第三医院接受治疗的118例输尿管超上段结石患者的临床资料，收集结石清除率、结石逃逸率、手术时间、术后住院时间等相关数据。结果:共118例纳入研究，总计101例(85.6%)结石未出现逃逸，通过输尿管硬镜联合N-trap完成手术，中位手术时间57(42,81) min,中位术后住院时间2(1,2)d,中位术后肌酐下降8(2,17)μmol/L,结石清除率为93.1%(94/101);17例(14.4%)结石出现逃逸，其中10例(58.8%,10/17)应用N-trap捞回后通过硬镜完成手术，中位手术时间76(60,97) min,中位术后住院时间2(2,2)d,中位术后肌酐下降16(11,22)μmol/L,结石清除率为80.0%(8/10)。结论:输尿管硬镜联合N-trap治疗输尿管超上段结石可有效减少结石逃逸，提高结石清除率，值得临床推广使用。更多还原

【Abstract】 Objective: To investigate the effect and experience of stone-resistance basket(N-trap) with rigid ureteroscopy in the treatment of hyper-upper ureteral calculi. Methods: A total of 118 patients with hyper-upper ureteral calculi were enrolled in this study from June 2021 to August 2024, and data related to stone removal rate, stone escape rate, operation time and postoperative hospital stay were collected. Results: A total of 118 patients were included in the study. A total of 101(85.6%) had no stone escape, and the operation was completed by rigid ureteroscope with N-trap with the median operation time of 57(42, 81) min, the median postoperative hospital stay of 2(1, 2) days, the median postoperative decrease in creatinine of 8(2, 17) μmol/L and the stone removal rate of 93.1%(94/101). Seventeen cases(14.4%) had stone escape, of which 10 cases(58.8%, 10/17) were retrieved by N-trap and then completed by rigid ureteroscope with the median operative time of 76(60, 97) min, the median postoperative hospital stay of 2(2, 2) days, the median postoperative decrease in creatinine of 16(11, 22) μmol/L and the stone removal rate of 80.0%(8/10). Conclusion: Rigid ureteroscopic lithotripsy combined with N-trap for the treatment of hyper-upper ureteral calculi could effectively reduce stone escape rate and improve the stone removal rate, which is worthy of clinical use.更多还原
