女性神经性贪食症患者脑岛静息态功能连接研究
Study on resting-state functional connectivity of the insula in female patients with bulimia nervosa
【摘要】 目的:探究女性神经性贪食症(bulimia nervosa,BN)患者岛叶静息态功能连接特征。方法:纳入27例女性BN患者(BN组)与相匹配的31名健康对照者(对照组)进行功能性核磁共振(functional magnetic resonance imaging,fMRI)扫描,基于人类脑网络组图谱中12个岛叶亚区为种子点进行全脑功能连接分析,使用进食障碍问卷(eating disorder inventory,EDI-Ⅰ)、焦虑自评量表(self-rating anxiety scale,SAS)和抑郁自评量表(self-rating depression scale,SDS)评估症状,Pearson相关分析探讨异常功能连接与症状的相关性。结果:与健康对照组比较,BN组右侧超颗粒岛叶和小脑蚓部的功能连接降低(t=-4.87),并与EDI-Ⅰ完美主义因子分负相关(r=-0.405);左腹侧无颗粒岛叶和左侧内侧前额叶皮质功能连接降低(t=-5.36),右腹侧无颗粒岛叶和左侧额中回功能连接增加(t=6.19);差异均有统计学意义(P均<0.05)结论:岛叶亚区与小脑蚓部、左侧内侧前额叶和左侧额中回功能连接异常可能是BN患者贪食行为和完美主义特征的神经病理基础。
【Abstract】 Objective: To explore the resting-state functional connectivity characteristics of the insula in female patients with bulimia nervosa(BN). Method: Resting state functional magnetic resonance imaging(rs-fMRI) data were acquired for 27 female patients with BN(BN group) and 31 matched healthy controls(control grouop). Whole-brain functional connectivity was analyzed using 12 insular subregions as seed points based on the human brainnetome atlas. Patients’ symptoms were evaluated by the eating disorder inventory(EDI-Ⅰ),self-rating anxiety scale(SAS),and self-rating depression scale(SDS). Pearson correlation analysis was conducted to explore the correlation between abnormal functional connectivity and symptoms. Results: Compared to the HC group,the BN group showed a decreased functional connectivity between the right hypergranular insula and the cerebellar vermis(t =-4. 87) and negatively correlated with the EDI-Ⅰperfectionism factor(r =-0. 405). The BN group also showed decreased functional connectivity between the left ventral agranular insula and the left medial prefrontal cortex(t =-5. 36) and an increased functional connectivity between the right ventral agranular insula and the left middle frontal gyrus(t = 6. 19). The difference is statistically significant(all P < 0. 05). Conclusion: The abnormal functional connectivity between the subregions of insula and the cerebellar vermis,left medial prefrontal cortex,and left middle frontal gyrus may be the underlying neuropathology maintaining binge eating and perfectionism seeking behaviors among BN patients.
