【摘要】 目的 比较比基尼入路(Bikini入路)和直接前方入路(DAA入路)行全髋关节置换术(THA)治疗的效果。方法 将58例患者按照入路不同分为DAA组(采用DAA入路行THA治疗,28例)和Bikini组(采用Bikini入路行THA治疗,30例)。比较两组围手术期指标、术后并发症发生情况、疼痛VAS评分、6 min步行距离、髋臼前倾角、外展角、髋臼覆盖率,采用Harris评分评价髋关节功能。结果 患者均获得3个月随访。术中出血量、手术时间、切口长度、住院时间、住院费用Bikini组均少(短)于DAA组(P<0.05)。术后3个月疼痛VAS评分、6 min步行距离、Harris评分Bikini组均优于DAA组(P <0.05)。术后3个月髋臼前倾角、外展角、髋臼覆盖率两组比较差异均无统计学意义(P> 0.05)。术后并发症发生率Bikini组低于DAA组(P<0.05)。结论 与DAA入路相比,Bikini入路行THA治疗具有切口小、损伤少、疼痛减轻、并发症发生率低、髋关节功能恢复效果佳等优势,疗效满意。更多还原

【Abstract】 Objective To compare the therapeutic effects of total hip arthroplasty(THA) performed via the Bikini approach and the direct anterior approach(DAA).Methods The 58 patients were divided into DAA group(THA was performed using DAA approach in 28 cases) and Bikini group(THA was performed using Bikini approach in 30cases) according to different approaches.The perioperative indicators,the incidence of postoperative complications,pain VAS,6-minute walking distance,acetabular anteversion angle,abduction angle,and acetabular coverage rate were compared between the two groups.Hip joint function was evaluated using the Harris score.Results All patients were followed up for 3 months.The intraoperative bleeding volume,surgical time,incision length,hospitalization time,and hospitalization expenses in the Bikini group were all less(shorter) than those in the DAA group(P <0.05).At 3months postoperatively,the pain VAS,6-minute walking distance,and Harris score in the Bikini group were all better than those in the DAA group(P <0.05).There were no significant differences in acetabular anteversion angle,abduction angle,and acetabular coverage rate between the two groups at 3 months postoperatively(P> 0.05).The incidence of postoperative complications in the Bikini group was lower than that in the DAA group(P <0.05).Conclusions Compared with the DAA,the Bikini approach for THA offers advantages such as smaller incision,reduced tissue damage,alleviated pain,lower incidence of complications,and superior hip joint function recovery,with satisfactory therapeutic efficacy.更多还原
