【摘要】 目的 评价经导管动脉栓塞术（transcatheter arterial embolization,TAE）治疗儿童血流动力学不稳定腹盆部外伤的安全性和有效性。方法 回顾性分析2016年5月至2025年1月在安徽省儿童医院接受急诊TAE治疗的19例血流动力学不稳定腹盆部外伤患儿。对血管造影结果进行评估，记录所用材料、手术数据、临床结果以及并发症。结果 19例患儿诊断性血管造影显示，单一脏器或部位出血13例，两处出血6例，所有出血源均被成功栓塞。使用弹簧圈实现栓塞13例，其中7例为弹簧圈混合其他栓塞材料。TAE联合其他手术2例。术后发生并发症5例，其中4例胆瘘，1例尿囊；死亡1例。结论 TAE治疗血流动力学不稳定腹盆部外伤患儿急性出血安全有效，并发症发生率相对较低。更多还原

【Abstract】 Objective To evaluate the safety and efficacy of transcatheter arterial embolization(TAE)in treating abdominal and pelvic trauma in pediatric patients with unstable hemodynamics.Methods The clinical data of 19 pediatric patients with abdominal and pelvic trauma and unstable hemodynamics,who received emergency TAE at the Anhui Provincial Children′s Hospital of China from May 2016 to January 2025,were retrospectively analyzed.The results of angiography were evaluated,and the used materials,surgical data,clinical outcomes,and complications were recorded.Results Diagnostic angiography showed that single organ or single site bleeding was found in13 patients(13/19,68.42%),bleeding at two sites was seen in 6 patients(6/19,31.58%).All the bleeding sources were successfully embolized.Spring coils were used in 13 patients(13/19),among them additional other embolization materials were employed in7 patients.TAE combined with other operation was carried out in2 patients.Postoperative complications occurred in 5 patients(5/29,26.32%),including biliary fistula(n=4)and urinoma formation(n=1).One patient died.Conclusion For the treatment of abdominal and pelvic trauma in pediatric patients with unstable hemodynamics,TAE is clinically safe and effective with a relatively low incidence of complications.更多还原
