【摘要】 目的：比较CT引导下Hookwire与微弹簧圈肺小结节定位技术在基层医院中的应用效果及安全性。方法：回顾性分析接受肺结节定位的109例患者的临床资料，其中Hookwire定位79例（Hookwire组），微弹簧圈定位30例（微弹簧圈组）。比较2组定位成功率、并发症发生率及定位时间的差异。结果：Hookwire组定位成功率为97.5%(77/79)，微弹簧圈组为100.0%(30/30)。Hookwire组术后气胸和出血的发生率分别为40.5%(32/79)和51.9%(41/79)，微弹簧圈组分别为43.3%(13/30)和50.0%(15/30)。2组定位体位、定位针肺内距离及并发症发生率比较，差异均无统计学意义（均P>0.05）;Hookwire组定位时间显著短于微弹簧圈组（P=0.033）。结论：CT引导下Hookwire与微弹簧圈为安全有效的肺结节定位方法，均有助于术中精准切除。Hookwire定位效果与微弹簧圈相似，且操作简便、成本较低，在基层医院中更具有潜在的临床推广价值。更多还原

【Abstract】 Objective:To compare the efficacy and safety of CT-guided Hookwire versus microcoil localization for pulmonary nodules in grass-roots hospitals. Methods:A retrospective analysis was conducted on 109 patients who underwent CT-guided pulmonary nodules localization. Among them,79 patients received Hookwire localization(Hookwire group) and 30 patients received microcoil localization(microcoil group). The localization success rate,complication rate and localization time of the two groups were compared. Results:The localization success rate was 97.5%(77/79) in the Hookwire group and 100.0%(30/30) in the microcoil group. The incidence rates of pneumothorax and pulmonary hemorrhage were 40.5%(32/79) and 51.9%(41/79) in the Hookwire group,compared to 43.3%(13/30) and 50.0%(15/30) in the microcoil group. There were no significant differences in patient position,intrapulmonary needle path length,or complication rates between the two groups(all P>0.05). However,the localization time in the Hookwire group was significantly shorter than that in the microcoil group(P=0.033). Conclusions:Both CT-guided Hookwire and microcoil localization are safe and effective methods for localization of pulmonary nodules,and are help to facilitate precise surgical excision. In grass-roots hospitals,Hookwire is not inferior to microcoil and offers advantages such as ease of operation and lower cost,making it more suitable for broader clinical application.更多还原
