基于“肺为血脏”探讨从“瘀”辨治肺动脉高压

【摘要】 肺动脉高压(PH)为进行性肺血管疾病,其发病率逐年升高,严重者可引发右心衰竭甚至死亡,现代医学缺乏针对性治疗手段,而中医药整体辨治治疗PH,可发挥协同作用,疗效确切。张伟教授基于中医古籍及临床诊治经验首次提出“肺为血脏”理论,即肺脏对血液的化生、输注有着不可替代的作用。文章基于该理论从“血瘀”入手,针对病机进行辨证论治,认为疾病初期,应以调气活血为主,辨证应用行气与补气,重在恢复肺对“气”的调节作用;疾病中期,应以活血化瘀为主,重在恢复肺对“血”的调节作用;疾病晚期,应以活血化瘀、利水逐饮为主,兼以补气,重在恢复肺对“气、血、水”的调节作用。

【Abstract】 Pulmonary hypertension(PH),as a progressive pulmonary vascular disease,has been increasing in incidence year by year.In severe cases,it can lead to right heart failure or even death.Modern medicine lacks targeted treatment options,while TCM takes a holistic approach and has demonstrated definite efficacy in treating PH,playing a crucial synergistic role.Prof.ZHANG Wei proposed the theory of "lung as the blood organ" for the first time,based on ancient TCM texts and clinical experience,suggesting that the lungs play an irreplaceable role in blood generation and circulation.Building on this theory,this paper focuses on "blood stasis" and implements syndrome differentiation and treatment based on pathogenesis.It is believed that in the early stages of the disease,the primary focus should be on regulating qi and activating blood circulation,with the application of qi-regulating and qi-tonifying therapies to restore the lung’ s regulatory function over "qi".In the intermediate stages,the emphasis shifts to activating blood circulation and resolving stasis,aiming to restore the lung’ s regulation of "blood".In the advanced stages,treatment should prioritize activating blood circulation,resolving stasis,promoting diuresis,and eliminating fluid retention,while also incorporating qi-tonifying measures,with the goal of restoring the lung’ s regulatory function over “qi,blood,and fluid".
