【摘要】 目的 分析595 nm脉冲染料激光(pulsed dye laser,PDL)治疗儿童面部鲜红斑痣的临床疗效，探讨影响其疗效的因素。方法 2010年10月—2020年10月纳入385例就诊于河南省人民医院皮肤科的儿童面部鲜红斑痣患者，进行595 nm PDL治疗，根据患儿性别、年龄、皮损类型、治疗次数、皮损面积将其分组，按Goh疗效评价标准分析各组有效率。并采用有序多分类logistics回归进行多因素分析，讨论上述各因素对疗效的影响。同时对相同起始治疗年龄和相同治疗次数的患儿分别进行有序多分类logistics回归，并探讨其各自的影响因素。结果 595 nm PDL治疗儿童面部鲜红斑痣总有效率为85.45%(329/385);术后不良反应发生率为4.42%(17/385)。随着起始治疗年龄的增加，疗效降低;紫红型皮损疗效比粉红型皮损疗效差;治疗次数增加，疗效增加;皮损面积增加，疗效降低。起始治疗年龄越小，首次治疗效果越好;治疗3次及7次时，皮损类型是影响疗效的因素;治疗5次时，年龄越小的粉红型皮损，效果越好。结论 595 nm PDL治疗儿童面部鲜红斑痣的疗效主要受起始治疗年龄、皮损类型、治疗次数及皮损面积影响。更多还原

【Abstract】 Objective To analyze the clinical efficacy of 595 nm pulsed dye laser( PDL) treatment for children with facial port-wine stains and explore the factors that affect its efficacy.Methods From October 2010 to October2020,a total of 385 children with facial port-wine stains who visited the dermatology department of Henan Provincial People’s Hospital were recruited and treated with 595 nm PDL.The patients were grouped according to their gender,age,skin lesion type,treatment frequency,and skin lesion area,and the effective rate of each group was analyzed according to the Goh efficacy evaluation criteria.And use ordered multi class logistics regression for multi factor analysis to discuss the impact of the above factors on the efficacy.Simultaneously,perform ordered multi class logistic regression on children with the same starting age and the same number of treatments,and explore their respective influencing factors.Results The total effective rate of 595 nm PDL treatment for children’s facial portwine stains was 85.45 %( 329/385); the incidence of postoperative side effects was 4.42 %( 17/385).As the age of initial treatment increases,the efficacy decreases; the therapeutic effect of purple red skin lesions was worse than that of pink skin lesions; An increase in the number of treatments leads to an increase in therapeutic efficacy;the area of skin lesions increases,and the therapeutic effect decreases.The younger the age of initial treatment,the better the initial treatment effect; the type of skin lesion was a factor that affects the therapeutic effect when treated 3 and 7 times; when treated 5 times,the younger the pink skin lesions, the better the effect.Conclusion The efficacy of 595 nm PDL treatment for children’s facial port-wine stains is mainly affected by the age of initial treatment,type of skin lesion,number of treatments and skin lesion area.更多还原
