【摘要】 目的 探讨2型糖尿病(T2DM)患者血液中维生素C(VC)的水平,为T2DM的病因和疾病预防提供参考依据。方法 通过Web of Science、PubMed、万方、中国知网(CNKI)数据库全面检索1980年1月1日至2024年2月26日公开发表的文献,使用纽卡斯尔-渥太华量表(NOS)对纳入文献进行质量评估;采用统计软件Stata 15.1对纳入的文献进行meta分析。结果 最终纳入符合标准的文献11篇,均为病例对照研究。病例组和对照组分别累计424例和393例;meta分析结果显示,病例组血VC水平较正常对照组低(WMD=-23.76,95%CI:-30.24~-17.28,P<0.001);meta回归分析显示,对照来源对研究间血VC水平异质性的影响有统计学意义(P<0.05);亚组分析显示,不同的洲际、国家、发表年份、对照来源、血样类型、样本量、血VC测定方法以及是否摄入VC制剂的研究中,病例组血VC水平均低于对照组(均P<0.05);敏感性分析显示纳入文献较为稳定;Begg’s检验提示本研究不存在发表偏倚(Z=1.25,P=0.213)。结论 T2DM患者血VC水平低于健康对照人群,推测VC摄入水平低可能是T2DM的病因之一,适量补充VC可能降低T2DM的发病风险。

【Abstract】 Objective To explore the levels of vitamin C(VC) in the blood of type 2 diabetes mellitus(T2DM) patients, and to provide reference for the etiology and prevention of T2DM. Methods A comprehensive search was conducted through the Web of Science, PubMed, Wanfang, and China National Knowledge Infrastructure(CNKI) databases for literature published between January 1, 1980, and February 26, 2024. The Newcastle-Ottawa scale(NOS) was used to assess the quality of the included literature. Meta-analysis was performed on the included literature using Stata 15.1 statistical software. Results A total of 11 case-control studies were ultimately included, involving 424 cases in the patient group and 393 in the control group. The meta-analysis results showed that the blood VC levels in the patient group were lower than those in the control group(WMD=-23.76, 95% CI:-30.24--17.28, P<0.001). Meta-regression analysis indicated that the source of controls had a statistically significant impact on the heterogeneity of blood VC levels among studies(P<0.05). Subgroup analysis revealed that in studies with different continents, countries, publication years, sources of controls, blood sample types, sample sizes, blood VC measurement methods, and whether VC supplements were taken, the blood VC levels in the patient group were significant lower than those in the control group(P<0.05). Sensitivity analysis showed that the included literature was relatively stable. Begg’s test suggested no publication bias in this study(Z=1.25, P=0.213). Conclusion The blood VC levels of T2DM patients are lower than those of healthy controls. It is speculated that low VC intake might be one of the causes of T2DM, and adequate supplementation of VC might decrease the risk of T2DM.
