2018—2023年河南省居民糖尿病疾病负担趋势分析

【摘要】 目的 分析2018—2023年河南省居民糖尿病疾病负担相关指标的变化趋势,为制定针对性防控策略提供依据。方法 利用河南省死因监测系统数据,计算糖尿病粗死亡率、年龄标化死亡率、早死概率、寿命损失年(YLL)率及平均寿命损失年(AYLL)等指标,并通过计算年度变化百分比(APC)分析趋势。结果 2018—2023年,河南省居民糖尿病粗死亡率从10.84/10万上升至21.16/10万,年龄标化死亡率从9.52/10万上升至14.25/10万,APC分别为15.77%和8.72%(t值分别为5.55和2.99,P均<0.05)。男性与女性糖尿病粗死亡率分别以年均17.69%和13.93%的速度上升(t值分别为5.09和6.18,P均<0.05),其中男性粗死亡率、年龄标化死亡率的上升速度均高于女性。各年龄组糖尿病死亡率随年龄增长而升高,其中0~<70岁各年龄组死亡率均呈上升趋势(t值分别为2.95、3.23、5.70、4.91和4.34,P均<0.05)。糖尿病所致早死概率从0.40%上升至0.59%,APC为9.40%(t=3.67,P<0.05)。YLL率从2.46‰上升至4.50‰,APC为13.71%(t=4.78,P<0.05);AYLL从22.65年下降至21.27年,APC为-1.23%(t=-16.87,P<0.05)。结论 河南省居民糖尿病死亡负担持续加重,其中男性和30~<60岁人群的死亡率增长速度更快。建议将上述人群列为防控的优先对象,实施靶向行为干预与全周期健康管理,并健全监测体系,为精准决策提供依据。

【Abstract】 Objective To analyze the changing trends of diabetes mellitus(DM) disease burden indicators among residents in Henan Province from 2018 to 2023, so as to provide evidence for developing targeted prevention and control strategies. Methods Data were obtained from Henan Provincial Cause-of-Death Surveillance System. The indicators including crude mortality rate,age-standardized mortality rate, premature mortality probability, years of life lost(YLL) rate, and average years of life lost(AYLL) due to DM were calculated, and the trends were analyzed by calculating the annual percentage change(APC). Results From 2018 to 2023, the crude mortality rate of DM among residents in Henan Province increased from 10.84/100 000 to 21.16/100 000, and the age-standardized mortality rate increased from 9.52/100 000 to 14.25/100 000, with APCs of 15.77% and8.72%, respectively(t =5.55 and 2.99, respectively; both P <0.05). The crude mortality rates in males and females increased annually by 17.69% and 13.93%, respectively(t=5.09 and 6.18, respectively; both P<0.05). Males showed faster increases in crude and standardized mortality rates than females. DM mortality increased with age, while significant upward trends were observed in all age groups under 70 years(t=2.95, 3.23, 5.70, 4.91 and 4.34, respectively; all P<0.05). The premature mortality probability due to DM rose from 0.40% to 0.59%, with an APC of 9.40%(t=3.67, P<0.05). The YLL rate increased from 2.46‰to 4.50‰, with an APC of 13.71%(t=4.78, P<0.05), while AYLL decreased from 22.65 years to 21.27 years, with an APC of-1.23%(t=-16.87, P<0.05). Conclusions The burden of DM mortality among residents in Henan Province continues to increase, with males and people aged 30 to < 60 experiencing faster rising rates. These populations should be designated as priority groups for prevention, implementing targeted behavioral interventions and life-course health management, along with enhanced surveillance systems to support evidence-based and precise decision-making.
