基于人-环境-作业模式的康复训练联合良肢位摆放对脑卒中后偏瘫患者的康复研究
Rehabilitation study of rehabilitation training based on person-environment-occupation model combined with good limb position in post-stroke hemiplegic patients
【摘要】 目的 探讨基于人-环境-作业(PEO)模式的康复训练联合早期良肢位摆放管理对脑卒中后偏瘫(PSH)患者的康复效果,为PSH患者制定更科学的康复方案提供参考。方法 选取2024-01—12南阳市中心医院收治的216例PSH患者为研究对象,采用随机数表法分为常规康复组和PEO模式康复组,各108例。所有患者均进行早期良肢位摆放管理,常规康复组患者在此基础上进行早期患肢康复训练,PEO模式康复组患者采取基于PEO模式的康复训练。干预3个月后,比较2组康复效果、干预前后肢体功能康复情况[上下肢Fugl-Meyer运动功能评估量表(FMA-UE、FMA-LE)评分和脑卒中患者姿势控制量表(PASS)评分]、神经功能[美国国立卫生研究院卒中量表(NIHSS)评分]和日常生活自理能力[Barthel指数(BI)]。结果 干预3个月后,PEO模式康复组康复总有效率93.52%,高于常规康复组的83.33%(χ~2=5.474,P=0.019)。干预3个月后,PEO模式康复组FMA-UE、FMA-LE、PASS评分、BI评分均高于常规康复组,NIHSS评分低于常规康复组(P<0.05)。结论 基于PEO模式的康复训练联合早期良肢位摆放管理可更好地改善PSH患者的运动能力和肢体控制能力,减轻神经功能损伤,进一步提升临床疗效和日常生活自理能力。
【Abstract】 Objective To investigate the rehabilitation effects of rehabilitation training based on person-environment-occupation(PEO)model combined with early good limb position management in post-stroke hemiplegic(PSH)patients.Methods A total of 216 patients with PSH admitted to Nanyang Central Hospital from January 2024 to December 2024 were selected as the research subjects and randomly divided into routine rehabilitation group and PEO model rehabilitation group by random number table method,108 patients in each group. All patients received early good limb position management. The patients in the conventional rehabilitation group underwent early rehabilitation training for the affected limb on this basis,while the patients in the PEO mode rehabilitation group received rehabilitation training based on the PEO model. After 3 month of intervention,the rehabilitation effects,limb function rehabilitation before and after intervention(upper and lower limb Fugl-Meyer assessment(FMA-UE,FMA-LE) score and postural assessment scale for stroke patients(PASS)score),neurological function(National Institutes of Health stroke scale,NIHSS),and daily living ability(Barthel index,BI)were compared between the two groups.Results After 3 month of intervention,the total effective rate of rehabilitation in the PEO model rehabilitation group was higher than that in the routine rehabilitation group(93.52% vs 83.33%,χ~2=5.474,P=0.019). After 3 month of intervention,the FMA-UE,FMA-LE,PASS,and BI scores in the PEO model rehabilitation group were higher than those in the routine rehabilitation group,and the NIHSS score was lower than that in the routine rehabilitation training group(P<0.05).Conclusion Rehabilitation training based on PEO model combined with early good limb position management can better improve the motor and limb control abilities of PSH patients,reduce neurological dysfunction and further enhance clinical efficacy and daily self-care ability.
