【摘要】 目的 检索、评价与整合根治性膀胱全切患者术前营养预康复管理的相关证据，为临床医护人员开展根治性膀胱全切患者的术前营养支持提供循证依据。方法 系统检索UptoDate、JBI循证护理数据库、国内外各指南网站、BMJ、CochraneLibrary、PubMed、Embase、WOS、中国知网、万方、维普等国内外数据库中有关根治性膀胱全切患者术前营养预康复管理的证据，检索类型涵盖专家共识、指南、随机对照研究以及系统评价等。检索时限为2002年1月—2024年5月。由2名受过循证法教学系统培训的研究人员对纳入文献内容实施质量评价，并经由4名研究者对所纳入证据内容进行独立的翻译、提炼及整合。结果 本研究共整合18篇文献，其中专家共识4篇、系统评价4篇、指南5篇、文献综述5篇。从根治性膀胱全切患者的营养风险筛查与评估、营养支持途径、碳水化合物负荷、吸烟与饮酒、术前禁食与禁饮5个方面，总结21条证据。结论 证据应用时临床医护人员可依据根治性膀胱全切患者的自身文化背景、价值观念、所在医院软硬件设施设备及证据应用利益相关者的主观意愿、促进和阻碍因素等对最佳证据内容进行选择性应用，为患者提供全面、科学规范且有循证可依的术前营养管理方案。更多还原

【Abstract】 Objective To retrieve,evaluate and integrate the evidence related to preoperative nutritional pre-rehabilitation management of radical cystectomy patients,and to provide an evidence-based basis for clinical healthcare professionals to carry out preoperative nutritional support for radical cystectomy patients.Methods Evidence on preoperative nutritional prerehabilitation management for patients with radical cystectomy was systematically searched in domestic and international databases such as UptoDate,JBI Evidence-Based Nursing Database,domestic and international guideline websites,BMJ,CochraneLibrary,PubMed,Embase,WOS,CNKI,Wanfang,Wipo and the types of searches covered expert consensus,guidelines,randomised controlled studies,and systematic evaluations,randomised controlled studies,and systematic evaluations.The search period was from January 2002 to May 2024,and was conducted by two experts trained in evidencebased methods.Two researchers trained in evidence-based teaching systems conducted quality assessment of the included literature,and four researchers independently translated,refined,and integrated the included evidence.Results A total of18 papers were integrated in this study,including 4 expert consensus papers,4 systematic evaluations,5 guidelines,and 5literature reviews.Twenty-one pieces of evidence were summarised in five areas:screening and assessment of nutritional risk in patients with radical cystectomy,nutritional support pathways,carbohydrate loading,smoking and alcohol consumption,and preoperative fasting and abstinence from food and drink.Conclusion Clinical staff can selectively apply the best evidence based on the cultural background and values of radical cystectomy patients,the hardware and software facilities and equipment of the hospital,and the subjective willingness,facilitating and hindering factors of the evidence application stakeholders,in order to provide patients with a comprehensive,scientifically standardised and evidence-based preoperative nutritional management plan.更多还原
