口腔扁平苔藓患者免疫指标水平与病损分型研究

【摘要】 目的 探究口腔扁平苔藓(oral lichen planus,OLP)患者辅助性T细胞17与调节性T细胞相关因子、免疫球蛋白A(immunoglobulin A,IgA)、免疫球蛋白G(immunoglobulin G,IgG)、免疫球蛋白M(immunoglobulin M,IgM)及补体成分3(complement 3,C3)、补体成分4(complement 4,C4)水平及其与病损分型的相关性。方法 纳入2021年9月—2022年10月在河北医科大学口腔医院口腔黏膜科就诊的OLP患者94例(OLP组,糜烂型50例,非糜烂型44例)及健康对照88例(对照组)。随机选取2组各15例检测其唾液中白细胞介素17(interleukin-17,IL-17)、白细胞介素23(interleukin-23,IL-23)和转化生长因子β1(transforming growth factor-β1,TGF-β1)水平。采集所有研究对象外周静脉血,利用酶联免疫吸附法检测IL-17、IL-23、TGF-β1水平,免疫比浊法测定免疫球蛋白及补体浓度。通过Spearman相关性分析研究各项指标与病损分型的关联。结果 与对照组相比,OLP患者唾液和血清中IL-17、IL-23水平显著升高,血清TGF-β1水平降低;血清IgA、IgG、C3水平升高,IgM、C4水平降低,差异有统计学意义(P<0.05)。相关性分析结果显示,IL-23与OLP病损分型呈正相关(P<0.05),且与糜烂型相关性(r=0.830)高于非糜烂型(r=0.765);C4与病损分型呈负相关(P<0.05),与糜烂型相关性(r=-0.853)高于非糜烂型(r=-0.826)。结论 OLP患者部分细胞因子及免疫指标异常,IL-23和C4与OLP病损分型密切相关,有望成为评估疾病严重程度的潜在生物标志物。

【Abstract】 Objective To investigate the levels of factors related to T helper 17(Th17) cells and regulatory T(Treg) cells, immunoglobulin A(IgA), immunoglobulin G(IgG), immunoglobulin M(IgM), complement 3(C3) and complement 4(C4) in patients with oral lichen planus(OLP), as well as their correlations with lesion classifications. Methods A total of 94 OLP patients(OLP group, including 50 cases of erosive type and 44 cases of non-erosive type) treated at the Department of Oral Mucosal Diseases, School and Hospital of Stomatology of Hebei Medical University from September 2021 to October 2022 and 88 healthy controls(control group) were enrolled. Fifteen subjects were randomly selected from each group to detect the salivary levels of interleukin-17(IL-17), interleukin-23(IL-23) and transforming growth factor-β1(TGF-β1). Peripheral venous blood samples were collected from all subjects. The levels of IL-17, IL-23 and TGF-β1 were detected by enzyme-linked immunosorbent assay(ELISA), while the concentrations of immunoglobulins and complements were determined by immunoturbidimetry. Spearman correlation analysis was performed to evaluate the association between each indicator and OLP lesion classification. Results Compared with the control group, OLP patients showed significantly elevated levels of IL-17 and IL-23 in saliva and serum, decreased serum TGF-β1, increased serum IgA, IgG and C3, and decreased serum IgM and C4, with significant differences(P<0.05). Correlation analysis revealed a positive correlation between IL-23 and OLP lesion classification(P<0.05), showing a stronger correlation with the erosive type(r=0.830) than with the non-erosive type(r=0.765). C4 was negatively correlated with lesion classification(P<0.05), showing a stronger correlation with the erosive type(r=-0.853) than with the non-erosive type(r=-0.826). Conclusion Several cytokines and immune indicators are abnormal in OLP patients, and IL-23 and C4 levels are closely related to OLP lesion classification, showing potential as biomarkers for assessing disease severity.
