CRIP2在前列腺癌中的表达失调与临床侵袭性及多西他赛化疗耐药的关系
Relationship of dysregulated CRIP2 expression in prostate cancer with clinical aggressiveness and docetaxel chemotherapy resistance
【摘要】 目的 探讨前列腺癌(PCa)组织中富含半胱氨酸肠蛋白2(CRIP2)表达情况与临床侵袭性及多西他赛(DTX)耐药的关系。方法 选取2021年5月至2023年12月南阳市中心医院收治的117例PCa患者,比较癌组织与癌旁组织中CRIP2 mRNA和蛋白表达情况,分析PCa癌组织CRIP2 m RNA和蛋白表达与临床病理特征的关系。所有患者接受DTX化疗,根据疗效分为DTX耐药组(n=31)与DTX敏感组(n=86),比较两组患者的基线特征、CRIP2m RNA和蛋白表达,采用Lasso-Logistic回归分析PCa患者DTX耐药的影响因素,受试者工作特征(ROC)曲线分析CRIP2蛋白单独及联合临床病理特征对DTX耐药的预测价值。结果 PCa患者癌组织CRIP2 m RNA和蛋白表达分别为2.16±0.56、2.05±0.37,明显高于癌旁组织的1.21±0.30、1.14±0.22,差异均有统计学意义(P<0.05);列联相关系数分析结果显示,PCa患者癌组织中的CRIP2 mRNA和蛋白表达与PSA、Gleason评分、淋巴结转移、临床分期、分化程度显著相关(P<0.05);DTX耐药组患者PSA≥20 ng/mL、临床分期Ⅳ期、淋巴结转移、Gleason评分≥8分占比分别为87.10%、90.32%、87.10%、83.87%,明显高于敏感组患者的68.60%、39.53%、50.00%、56.98%,差异均有统计学意义(P<0.05);DTX耐药组患者的CRIP2 m RNA和蛋白表达分别为2.42±0.27、2.18±0.20,明显高于DTX敏感组的2.07±0.22、2.00±0.16,差异均有统计学意义(P<0.05);Gleason评分、临床分期、CRIP2蛋白是DTX耐药的独立影响因素(P<0.05);CRIP2蛋白、Gleason评分、临床分期联合预测模型预测PCa患者DTX耐药的AUC为0.863,预测价值优于CRIP2蛋白单独预测价值(P<0.05)。结论 PCa癌组织中CRIP2 m RNA和蛋白表达上调,与患者病理特征、DTX耐药性有关,CRIP2蛋白表达可辅助用于评估PCa患者DTX耐药性。
【Abstract】 Objective To investigate the relationship between the expression of cysteine-rich intestinal protein 2(CRIP2) in prostate cancer(PCa) tissues and clinical aggressiveness as well as docetaxel(DTX) resistance. Methods A total of 117 PCa patients admitted to Nanyang Central Hospital from May 2021 to December2023 were selected. The expression of CRIP2 m RNA and protein was compared between cancer tissues and adjacent non-cancerous tissues. The relationship between CRIP2 mRNA and protein expression in PCa tissues and clinicopathological characteristics was analyzed. All patients received DTX chemotherapy and were divided into a DTX-resistant group(n=31) and a DTX-sensitive group(n=86) based on treatment response. Baseline characteristics, CRIP2 m RNA and protein expression were compared between the two groups. Lasso-Logistic regression was used to analyze the influencing factors of DTX resistance in PCa patients. Receiver operating characteristic(ROC) curve was used to evaluate the predictive value of CRIP2 protein alone and in combination with clinicopathological characteristics for DTX resistance. Results The expression of CRIP2 mRNA and protein in cancer tissues of PCa patients was 2.16±0.56 and 2.05±0.37, respectively, which were significantly higher than 1.21±0.30 and 1.14±0.22 in adjacent non-cancerous tissues(both P<0.05). Contingency correlation coefficient analysis showed that CRIP2 mRNA and protein expression in cancer tissues of PCa patients were significantly correlated with PSA level, Gleason score, lymph node metastasis, clinical stage,and degree of differentiation(P<0.05). The proportions of patients with PSA≥20 ng/mL, clinical stage Ⅳ, lymph node metastasis, and Gleason score≥8 in the DTX-resistant group were 87.10%, 90.32%, 87.10%, and 83.87%, respectively,which were significantly higher than 68.60%, 39.53%, 50.00%, and 56.98% in the DTX-sensitive group(all P<0.05).The expression of CRIP2 m RNA and protein in the DTX-resistant group was 2.42±0.27 and 2.18±0.20, respectively,which were significantly higher than 2.07±0.22 and 2.00±0.16 in the DTX-sensitive group(both P<0.05). Gleason score,clinical stage, and CRIP2 protein were independent influencing factors for DTX resistance(P<0.05). The combined prediction model of CRIP2 protein, Gleason score, and clinical stage had an AUC of 0.863 for predicting DTX resistance in PCa patients, and its predictive value was superior to that of CRIP2 protein alone(P<0.05). Conclusion CRIP2 mRNA and protein expression are upregulated in PCa tissues, which is associated with patient pathological characteristics and DTX resistance. CRIP2 protein expression can be used as an auxiliary indicator to assess DTX resistance in PCa patients.
