CRIP2表达预测前列腺癌患者多西他赛耐药的价值

【摘要】 目的 探讨富含半胱氨酸肠蛋白2(CRIP2)表达预测前列腺癌多西他赛耐药性的价值。方法 前瞻性选取2022年1月至2024年1月于南阳市中心医院就诊的164例前列腺癌患者,所有患者均接受多西他赛治疗,治疗4个疗程(每28 d为一个疗程)后评价治疗效果,将99例有效者纳入敏感组,65例无效者纳入耐药组。比较两组患者的基本特征、CRIP2表达水平,采用多因素Logistic回归分析前列腺癌患者多西他赛耐药性的影响因素,采用受试者工作特征(ROC)曲线分析CRIP2表达预测多西他赛耐药性的价值。结果 耐药组患者的系统免疫炎症指数(SII)、微血管密度及CRIP2 mRNA和蛋白表达分别为1 000.12±300.34、(16.20±4.45)条/200倍视野、5.58±1.57、4.06±0.32,明显大于敏感组的705.56±211.89、(11.63±3.41)条/200倍视野、3.96±1.06、3.68±0.30,差异均有统计学意义(P<0.05);多因素Logistic回归分析结果显示,CRIP2 m RNA和蛋白表达、SII、肿瘤分期、肿瘤分化程度、淋巴结转移均是前列腺癌患者多西他赛耐药性的影响因素(P<0.05);ROC分析结果显示,CRIP2 m RNA、蛋白表达单一及联合预测多西他赛耐药性的AUC分别为0.723(95%CI:0.648~0.790)、0.746(95%CI:0.672~0.810)、0.863(95%CI:0.800~0.911),且联合预测的AUC高于单一指标预测(Z=2.705、2.436,P<0.05)。结论 前列腺癌多西他赛耐药性患者癌组织中CRIP2 m RNA和蛋白表达水平升高,检测其水平对多西他赛耐药性具有一定预测价值,联合检测其水平可提高预测效能。

【Abstract】 Objective To investigate the value of cysteine-rich intestinal protein 2(CRIP2) expression in predicting docetaxel resistance in prostate cancer patients. Methods A prospective study was conducted on 164 prostate cancer patients treated at Nanyang Central Hospital from January 2022 to January 2024. All patients received docetaxel treatment. Therapeutic efficacy was evaluated after 4 treatment cycles(28 days per cycle). Ninety-nine patients who responded were included in the sensitive group, and 65 non-responders were included in the resistant group. Basic characteristics and CRIP2 expression levels were compared between the two groups. Multivariate logistic regression analysis was used to identify factors influencing docetaxel resistance. Receiver operating characteristic(ROC) curve analysis was performed to assess the value of CRIP2 expression in predicting docetaxel resistance. Results The systemic immune-inflammation index(SII), microvessel density, and CRIP2 mRNA and protein expression levels in the resistant group were 1 000.12±300.34,(16.20±4.45) vessels/200×field, 5.58±1.57, and 4.06±0.32, respectively, significantly higher than 705.56±211.89,(11.63±3.41) vessels/200×field, 3.96±1.06, and 3.68±0.30 in the sensitive group(all P<0.05).Multivariate logistic regression analysis indicated that CRIP2 m RNA and protein expression, SII, tumor stage, tumor differentiation degree, and lymph node metastasis were all influencing factors for docetaxel resistance in prostate cancer patients(P<0.05). ROC analysis showed that the AUCs for predicting docetaxel resistance using CRIP2 m RNA alone, protein alone, and their combination were 0.723(95% CI: 0.648-0.790), 0.746(95% CI: 0.672-0.810), and 0.863(95% CI:0.800-0.911), respectively. The AUC for the combined prediction was significantly higher than that for either single indicator(Z=2.705, 2.436, P<0.05). Conclusion CRIP2 mRNA and protein expression levels are elevated in the cancerous tissues of prostate cancer patients with docetaxel resistance. Detecting these levels has certain predictive value for docetaxel resistance, and combined detection can improve predictive efficacy.
