结直肠癌组织中miR-383-5p和KIF3B的表达及临床意义
Expression and clinical significance of miR-383-5p and KIF3B in colorectal cancer tissues
【摘要】 目的 探究结直肠癌(CRC)组织中mi R-383-5p、驱动蛋白家族成员3B(KIF3B)的表达及临床意义。方法选择2017年7月至2020年6月于保定市第一中心医院接受根治性手术治疗的85例CRC患者作为研究对象,术中获取患者的CRC组织和癌旁组织(距离肿瘤边缘>5 cm)标本。采用实时荧光定量PCR法检测组织中miR-383-5p、KIF3B mRNA表达水平。采用免疫组织化学染色法检测KIF3B蛋白表达情况。采用Pearson相关分析评估CRC组织中miR-383-5p与KIF3B的相关性。采用Kaplan-Meier生存曲线分析CRC组织中miR-383-5p、KIF3B与患者预后的关系。采用Cox比例风险回归模型分析CRC患者预后的影响因素。结果 与癌旁组织相比,CRC组织中miR-383-5p表达水平降低,KIF3B mRNA表达水平和蛋白阳性表达率均升高(P均<0.05)。Pearson相关分析结果显示,CRC组织中miR-383-5p与KIF3B表达水平呈负相关(r=-0.484,P<0.001)。miR-383-5p低表达组的3年生存率(72.73%)显著低于mi R-383-5p高表达组(90.24%),KIF3B阴性表达组的3年生存率(92.86%)显著高于KIF3B阳性表达组(69.77%),差异均有统计学意义(P均<0.05)。多因素Cox回归分析结果显示,KIF3B阳性表达是CRC患者术后3年预后的独立危险因素,而mi R-383-5p高表达是独立保护因素(P均<0.05)。结论 CRC组织中miR-383-5p表达下调,KIF3B表达上调,两者均与患者预后有关,均有潜力作为CRC预后评估的生物标志物。
【Abstract】 Objective This paper aims to investigate the expression levels and clinical significance of miR-383-5p and kinesin family member 3B(KIF3B) in colorectal cancer(CRC) tissues. Methods A total of 85 patients with CRC who underwent radical surgery at Baoding First Central Hospital from July 2017 to June 2020 were enrolled. CRC tissue specimens and adjacent normal tissue specimens(more than 5 cm from the tumor edge) were obtained intraoperatively. Real-time fluorescence quantitative polymerase chain reaction(PCR) was used to detect the mRNA expression levels of miR-383-5p and KIF3B in tissues. Immunohistochemical staining method was performed to examine the protein expression level of KIF3B. Pearson correlation analysis was applied to explore the relationship between miR-383-5p and KIF3B in CRC tissues. Kaplan-Meier survival curves were used to analyze the relationship between miR-383-5p and KIF3B expression in CRC tissues and patient prognosis. The Cox proportional hazards regression model was used to identify prognostic factors in CRC patients. Results Compared with adjacent normal tissues, CRC tissues showed significantly decreased expression of miR-383-5p, and increased mRNA expression level and positive protein expression rate of KIF3B(P<0.05). Pearson correlation analysis showed that miR-383-5p was negatively correlated with KIF3B in CRC tissues(r=-0.484, P<0.001). The 3-year survival rate was significantly lower in the low miR-383-5p expression group(72.73%) than in the high miR-383-5p expression group(90.24%), and significantly higher in the KIF3B-negative expression group(92.86%) than in the KIF3B-positive expression group(69.77%), with statistically significant differences(P<0.05). Multivariate Cox regression analysis showed that positive KIF3B expression was an independent risk factor for 3-year postoperative prognosis in CRC patients, whereas high expression of miR-383-5p was an independent protective factor(P<0.05). Conclusion miR-383-5p was downregulated and KIF3B is upregulated in CRC tissues, both of which are associated with patient prognosis and may serve as biomarkers for prognostic evaluation of CRC.
