【摘要】 目的 ：探讨黄斑变性患者玻璃体腔注药依从性影响因素及生成机制，为提高其治疗依从性，改善视力结局及生活质量的干预研究提供理论依据。方法 ：基于扎根理论，整理分析33名受访者的访谈资料，进行开放式编码、主轴编码和选择性编码，构建黄斑变性患者玻璃体腔注药依从性影响机制理论框架，使用SPSS Modeler中Apriori算法建模，进行数据挖掘得出范畴内部数据关联。结果 ：黄斑变性患者玻璃体腔注药行为遵循“原因-现象-情境条件-干预条件-行动/互动-结果”的逻辑行为过程，患者相关因素直接影响其注药依从行为，疾病与治疗相关因素为内部情境因素，医疗服务系统和社会经济因素为外部情境因素，对患者依从行为起调节作用。通过数据挖掘得出3项强规则关联因素组合。结论 ：医护人员应及时评估影响患者注药的阻碍与促进因素，加强医患沟通、提升医护人员健康教育水平并发挥家庭支持作用，改善患者依从性及注药治疗效果。更多还原

【Abstract】 Objective: To explore the influencing factors and generating mechanisms of intravitreal injection adherence in patients with macular degeneration, so as to provide a theoretical basis for intervention studies to improve patients’ adherence, visual outcomes and quality of life. Methods: Based on grounded theory, we interviewed 33 participants to collect data. Then a theoretical framework for the influencing mechanism of intravitreal administration adherence in patients with macular degeneration was established by means of open coding, spindle coding and selective coding. And data mining using the Apriori algorithm in SPSS Modeler was performed to further determine the relationships between the data. Results: Patients with macular degeneration receiving intravitreal injection were found to observe the logical behavioural process of "cause-phenomenon-situational conditions-intervention conditions-action/interaction-results". According to the results, patient-related factors directly influenced their injection compliance, with disease-and treatment-related factors as internal situational factors, and healthcare system and socio-economic factors as external situational factors that could regulate patient adherence. Besides, three pairs of strong rule-based correlates were derived through data mining. Conclusion: Improving patients’ adherence to intravitreal injection requires more doctor-patient communication, enhanced health education by medical staff and adequate family support. By establishing a good family and social support environment, we can help patients overcome treatment barriers and improve their knowledge of and confidence in adherence to intravitreal injection therapy.更多还原
