Allg??wer-Donati缝合技术在全膝关节置换手术切口缝合中应用的效果观察
Observation on effect of Allg??wer-Donati suturing technique in incision suturing for total knee arthroplasty
【摘要】 目的 评估Allg??wer-Donati缝合技术在全膝关节置换手术切口缝合中的临床应用效果。方法 回顾性分析自2021-01—2023-12采用全膝关节置换术治疗的100例膝关节骨性关节炎,观察组50例切口采用Allg??wer-Donati缝合技术缝合,对照组50例切口采用垂直褥式缝合技术缝合。比较两组手术时间、切口缝合时间、切口引流时间、切口愈合时间、术后12周膝关节活动度、术后3个月切口瘢痕面积、术后切口并发症发生率,术后第3天、1周、2周疼痛VAS评分,以及术后3个月患者-观察者瘢痕评估量表。结果 观察组随访时间为(13.56±2.95)个月,对照组为(14.14±2.69)个月。观察组与对照组手术时间、切口缝合时间、切口引流时间、切口愈合时间、术后12周膝关节活动度、术后疼痛VAS评分差异无统计学意义(P>0.05)。观察组术后3个月切口瘢痕面积小于对照组,术后切口并发症发生率低于对照组,差异有统计学意义(P<0.05)。观察组中患者瘢痕评估量表总分、瘢痕厚度评分、瘢痕硬度评分、瘢痕颜色评分、瘢痕瘙痒评分低于对照组,差异有统计学意义(P<0.05);而两组皮肤规则评分及瘢痕疼痛评分差异无统计学意义(P>0.05)。观察组中观察者瘢痕评估量表总分及瘢痕表面积评分低于对照组,差异有统计学意义(P<0.05);而两组瘢痕血管分布评分、瘢痕色泽评分、瘢痕厚度评分、瘢痕粗糙度评分及瘢痕柔软度评分差异无统计学意义(P>0.05)。结论 全膝关节置换术中可以采用Allgower-Donati缝合技术缝合切口,有效减少了术后切口瘢痕面积,降低了切口并发症发生风险,提高了患者的满意度。
【Abstract】 Objective To evaluate the clinical application efficacy of the Allg??wer-Donati suture technique in wound closure for total knee arthroplasty.Methods A retrospective analysis was conducted on 100 patients with knee osteoarthritis treated by total knee arthroplasty from January 2021 to December 2023. In the observation group(n=50), the Allg??wer-Donati suture technique was used for wound closure, while the control group(n=50) underwent vertical mattress suture technique. The operative time, wound closure time, wound drainage duration, wound healing time, knee range of motion at 12 weeks postoperatively,scar area at 3 months postoperatively, incidence of wound complications, Visual Analogue Scale(VAS) pain scores on day 3, at week 1, and week 2 postoperatively, and Patient and Observer Scar Assessment Scale(POSAS) scores at 3 months postoperatively were compared between the two groups.Results The follow-up period was(13.56±2.95) months in the observation group and(14.14±2.69) months in the control group. No statistically significant differences were observed between the two groups in operative time, wound closure time, wound drainage duration, wound healing time, knee range of motion at 12 weeks postoperatively, and postoperative VAS pain scores(P>0.05). The observation group exhibited a significantly smaller scar area at 3 months postoperatively and a lower incidence of wound complications compared to the control group(P<0.05). In the Patient Scar Assessment Scale, the observation group demonstrated significantly lower total scores, as well as lower scores for scar thickness, hardness, color, and pruritus(P<0.05), while no significant differences were found in skin regularity or scar pain scores between the two groups(P>0.05). In the Observer Scar Assessment Scale, the observation group showed significantly lower total scores and surface area scores(P<0.05), whereas no significant differences were observed in scores for vascularity, pigmentation, thickness, relief, or pliability(P>0.05).Conclusion The Allg??wer-Donati suture technique is a viable option for wound closure in total knee arthroplasty, effectively reducing postoperative scar area, lowering the risk of wound complications, and improving patient satisfaction.
