基于CCTA识别的易损斑块与临床危险因素的相关性分析

【摘要】 目的 本研究通过冠状动脉计算机断层扫描(CCTA)探讨易损斑块的发生与临床危险因素的关系。方法 共有435例疑似CAD患者接受了CCTA检查,从CCTA辨别出易损斑块,并计算冠脉的FFR值,评估了年龄、性别、糖尿病、高血压、吸烟、饮酒、BMI值、CRP、LDL-C和Hb A1c等变量。使用Logstic回归分析与易损斑块发生的危险因素。结果 易损斑块的存在与HbA1c≥6.1%(P=0.04)、LDL-C≥3.1mmol/L(P=0.02)、FFR≤0.75(P=0.01)、糖尿病(P<0.01)及吸烟(P=0.04)相关;而与其他因素无关。Logistic回归分析进一步表明,多变量模型的AUC=0.77。结论 CCTA作为一种非侵入性工具在识别易损斑块具有潜力,多种临床危险因素与易损斑块发生相关,易损斑块与FFR≤0.75显著相关,提示易损斑块可能与冠脉血流动力学异常存在内在联系。

【Abstract】 Objective This study aimed to investigate the relationship between the occurrence of vulnerable plaques(VP) and clinical risk factors using co ronary computed tomography angiography(CCTA).Methods A total of 435 patients with suspected coronary artery disease(CAD) underwent CCTA.Vulnerable plaques were identified from CCTA images,and fractional flow reserve(FFR) values were calculated.Various clinical variables,including age,gender,diabetes,hypertension,smoking,alcohol consumption,BMI,CRP,LDL-C,and HbA1c,were assessed.Logistic regression was employed to analyze risk factors associated with the occurrence of VP.Results The presence of VP was significantly associated with HbA1c≥6.1%(p=0.04),LDL-C≥3.1 mmol/L(P=0.02),FFR ≤0.75(P=0.01),diabetes(P<0.01),and smoking(P=0.04),while no significant association was observed with other factors.Logistic regression analysis further revealed that the multivariable model achieved an AUC of 0.77.Conclusion CCTA,as a non-invasive tool,shows potential in identifying vulnerable plaques.The occurrence of VP is significantly associated with several clinical risk factors,including FFR ≤0.75,suggesting an intrinsic relationship between VP and coronary hemodynamic abnormalities.
