mDIXON-Quant序列衍生腰椎骨髓脂肪分数在2型糖尿病性骨质疏松症诊断中的应用价值
The Value of mDIXON-Quant Sequence-Derived Lumbar Bone Marrow Fat Fraction in the Diagnosis of Type 2 Diabetic Osteoporosis
【摘要】 目的 研究MRI中mDIXON-Quant序列衍生腰椎脂肪分数(FF)在2型糖尿病性骨质疏松症诊断中的应用价值。方法 收集2023年7月至2024年7月在我院就诊符合纳排标准的患者103名。采用双能X射线骨密度仪(DXA)检测骨密度值(T值),依据T值,将研究对象分为骨质正常组(A组)、骨量减少组(B组)和骨质疏松组(C组),采用磁共振成像(MRI)的mDIXON-Quant序列扫描获取脂肪分数图像,进行定量分析测得FF值,比较3组间FF值,并分析FF值与T值的相关性,采用ROC曲线评估FF值诊断2型糖尿病患者骨质疏松症的效能。结果 单因素方差分析结果提示,3组患者间FF差异具有统计学意义(P<0.05)。Spearman秩相关分析提示,FF与骨密度呈负相关(r=-0.846, P<0.01)。ROC曲线结果显示,FF值区分骨质正常与骨量减少时,[AUC(95%CI)=0.980(0.957~1.000), P<0.001],最佳截断值为60.07%,对应的灵敏度为88.9%,特异度为97.1%;FF值鉴别骨量减少与骨质疏松时,[AUC(95%CI)=0.955(0.903~1.000), P<0.001],最佳截断值为65.71%,对应的灵敏度为90.9%,特异度为97.2%。结论 mDIXON-Quant扫描技术可以帮助评估2型糖尿病患者腰椎骨髓脂肪的含量,为骨质正常、骨量减少和骨质疏松的早期诊断提供思路与方法。
【Abstract】 Objective To investigate the value of mDIXON-Quant sequence-derived lumbar fat fraction( FF) in MRI in the diagnosis of type 2 diabetic osteoporosis. Methods One hundred and three patients who attended our hospital from July 2023 to July 2024 and met the criteria for nadir were collected. Dualenergy X-ray bone densitometry(DXA) was used to detect the bone density value(T-value), and based on the T-value, the study subjects were divided into the normal bone group(Group A), the bone loss group(Group B), and the osteoporosis group(Group C), and the fat fraction images were obtained by scanning with the mDIXON-Quant sequence of magnetic resonance imaging(MRI) to quantitatively analyse the FF-value measured, to compare FF-value among the 3 groups, and to The correlation between FF values and T values was analysed, and ROC curves were used to assess the efficacy of FF values in diagnosing osteoporosis in type 2 diabetic patients. Results The results of one-way ANOVA suggested that the difference in FF between the three groups was statistically significant(P<0.05).Spearman rank correlation analysis suggested that FF was negatively correlated with bone mineral density(r=-0.846, P<0.01).The results of the ROC curve showed that the FF values differentiated between normal bone quality and reduced bone mass, [AUC(95%) CI) = 0.980(0.957-1.000), P<0.001], the optimal cut-off value was 60.07%, corresponding to a sensitivity of 88.9% and a specificity of 97.1%; when the FF value discriminated between reduced bone mass and osteoporosis, [AUC(95% CI) = 0.955(0.903~1.000), P<0.001], the optimal cut-off value was 65.71%, corresponding to a sensitivity of 90.9% and a specificity of 97.2%. Conclusion The mDIXON-Quant scanning technique can help to assess the content of lumbar spinal marrow fat in patients with type 2 diabetes mellitus, and provide ideas and methods for the early diagnosis of normal bone quality, bone loss and osteoporosis.
