国医大师周仲瑛基于整体观辨治失眠经验
Research using Data Mining Techniques to Study the Experience of National Master of Traditional Chinese Medicine ZHOU Zhongying in Treating Insomnia based on a Holistic Approach
【摘要】 目的:该研究基于周教授整体观理论,通过多数据技术分析周仲瑛教授诊疗失眠的整体性辨证框架,采用智能算法对诊疗数据进行特征解构与关联建模,系统揭示失眠的多维病理要素(涉及多维度致病机制、复合病位关联及动态病性演变规律),并分析复方组配过程中药效物质协同规律与剂量响应特征,为临床辨治失眠提供整合性诊疗策略。方法:该研究纳入1984年5月—2014年10月周教授门诊失眠诊疗病例,原始数据源自周教授传承工作室电子病案库。通过关键词“失眠”检索获取1360诊次病案,经标准化数据清洗流程处理,最终保留初诊病例484例、复诊诊次288例次。通过Excel 2019与R3.2.5实现人口学特征、症状体征、舌脉参数、病机要素的频次分布统计及布尔矩阵转换;应用IBM SPSS Statistics 22.0平台执行层次聚类算法,解析症状-病机关联特征;利用IBM SPSS Modeler 18.0的Apriori算法进行方药配伍关联规则挖掘;借助Liquorice构建核心症状-病机-方药复杂网络图谱,量化节点中心度参数。结果:周教授在失眠治疗中强调“复法制方”,即通过多靶点、多层次的药物组合调节整体功能。高频药物组合(如酸枣仁-黄连、夜交藤-知母)的关联规则分析显示,其置信度高达100.00%,体现了药物协同作用的科学性。核心方剂兼顾安神、清热、活血、滋阴等功效,既针对失眠的直接症状,又调节潜在的病理基础(如痰热、肝郁、阴虚等),体现了“标本同治”的整体观思想。结论:周教授论治失眠时立足于整体选方用药,进行整体调控。
【Abstract】 Objective:Based on Professor ZHOU’s holistic theory,this study analyzes Professor ZHOU’s holistic diagnostic framework for treating insomnia using multi-data technology.Intelligent algorithms are employed to deconstruct and model the clinical data,systematically revealing the multidimensional pathological elements of insomnia (involving multidimensional pathogenic mechanisms,complex lesion location associations,and dynamic pathological evolution patterns).The study also analyzes the synergistic relationship of pharmacodynamic substances and dose-response characteristics during compound prescription formulation,providing an integrated diagnostic and treatment strategy for insomnia in clinical practice.Methods:This study includes insomnia cases treated by Professor ZHOU at his outpatient clinic from May 1984 to October 2014.The original data comes from the electronic medical record database of Professor ZHOU’s Heritage Studio.A search using the keyword “insomnia” yielded 1360 medical records.After standardized data cleaning,484 initial consultation cases and 288 follow-up consultation cases were retained.The frequency distribution statistics of demographic characteristics,symptoms and signs,tongue and pulse parameters,and pathogenesis elements were statistically analyzed and Boolean matrix transformations were performed using Excel 2019 and R3.2.5.A hierarchical clustering algorithm was applied using IBM SPSS Statistics 22.0 to analyze the symptom-pathogenesis association features.The Apriori algorithm of IBM SPSS Modeler 18.0 was used to mine the association rules of drug combinations.A complex network graph of core symptoms-pathogenesis-drug combination was constructed using Liquorice,and the node centrality parameter was quantified.Results:Professor Zhou emphasized “complex prescription” in the treatment of insomnia,that is,regulating overall function through multi-target,multi-level drug combinations.Association rule analysis of high-frequency drug combinations (such as Ziziphus jujuba seed-Coptis chinensis,Polygonum multiflorum-Anemarrhena asphodeloides) showed a confidence level as high as 100.00%,reflecting the scientific nature of drug synergy.The core formula combines the effects of calming the mind,clearing heat,promoting blood circulation,and nourishing Yin.It addresses both the direct symptoms of insomnia and regulates the underlying pathological basis (such as phlegm-heat,liver stagnation,and Yin deficiency),embodying the holistic approach of “treating both the root cause and the symptoms.” Conclusion:Professor ZHOU’s treatment of insomnia is based on holistic selection of prescriptions and medications,aiming for overall regulation.
