不同方案预防中国高危孕妇子痫前期发生的网状Meta分析
Different strategies for preventing preeclampsia in high-risk pregnant women in China:a network meta-analysis
【摘要】 目的 采用网状Meta分析方法比较不同药物使用方案预防中国高危孕妇子痫前期发生的有效性。方法 计算机检索CNKI、WanFang Data、VIP、PubMed、Embase,Cochrane library数据库,搜集不同药物方案预防中国高危孕妇子痫前期发生的随机对照试验(RCT),检索时限均从2000年1月至2024年8月。由2位研究者根据纳入与排除标准独立筛选文献、提取资料和评价纳入研究后,采用Stata 15.0软件进行网状Meta分析。结果 共纳入33项RCT,包括5 364例患者,涉及5种药物干预方案。网状Meta分析结果显示,阿司匹林联合维生素和阿司匹林联合钙剂预防子痫前期发生的效果好于单用阿司匹林至孕36周。最佳概率排序结果显示,阿司匹林联合维生素和阿司匹林联合钙剂方案是预防子痫前期发生最优干预措施的概率更高。结论 阿司匹林联合维生素和阿司匹林联合钙剂对预防中国高危孕妇子痫前期发生可能效果最佳,但同时应根据患者的具体情况制定个体化的用药方案进行干预。
【Abstract】 Objective To investigate the effectiveness of different drug use regimens in preventing the development of preeclampsia in high-risk pregnant women in China. Methods We searched CNKI, WanFang Data, VIP,PubMed, Embase, and Cochrane library databases. Randomized controlled trials(RCTs) of published studies on the prevention of the onset of preeclampsia in high-risk pregnant women in China were searched between January 2000 and August 2024. After two investigators independently screened the literature according to the inclusion and exclusion criteria, extracted the data, and evaluated the included studies, a network meta-analysis was performed using Stata 15.0software. Results A total of 33 RCTs with 5 364 patients involving five pharmacological intervention regimens were included. Network meta-analysis showed that aspirin combined with vitamins and aspirin combined with calcium were more effective than aspirin alone up to 36 weeks in preventing the onset of preeclampsia. The results of the best probability ranking showed that aspirin combined with vitamins and aspirin combined with calcium had a higher probability of being optimal for preventing the onset of preeclampsia. Conclusion Aspirin is effective in preventing the development of preeclampsia in high-risk pregnant women in China, but it is necessary to develop an individual combination management plan according to the patient’s specific situation.
