布地奈德对支气管哮喘急性发作期患儿呼吸功能及细胞免疫的影响

【摘要】 目的:探讨布地奈德对支气管哮喘急性发作期患儿呼吸功能及细胞免疫的影响。方法:选取2020年3月~2021年3月在某院就诊的支气管哮喘急性发作期患儿130例,随机分为常规组(给予常规抗哮喘治疗)和治疗组(在常规组基础上应用布地奈德混悬液雾化吸入治疗),每组65例。比较两组患儿用药前后呼吸功能、血清炎症因子水平、患儿症状消退时间、T淋巴细胞亚群、临床治疗总有效率及不良反应总发生率。结果:治疗后,两组患儿第1秒用力呼气容积(FEV1)、用力呼出75%肺活量的呼气流量(FEF75)和第1秒用力呼气容积与用力肺活量比值(FEV1/FVC)及CD4+、CD4+/CD8+均升高,且治疗组高于常规组(P<0.05);血清炎症因子降钙素原(PCT)、超敏C反应蛋白(hs-CRP)、白介素-8(IL-8)及CD8+水平均降低,且治疗组低于常规组(P<0.05)。治疗组患儿临床症状(咳嗽、气喘、肺部湿啰音及呼吸困难)消退时间较常规组均明显缩短(P<0.05);临床治疗总有效率(92.31%)高于常规组(78.46%,χ2=4.993,P=0.025)。两组患者不良反应总发生率比较无统计学差异(χ2=0.769,P=0.380)。结论:支气管哮喘急性发作期患儿给予布地奈德混悬液雾化吸入后,临床症状显著缓解,呼吸功能及免疫功能得到明显改善。

【Abstract】 Objective: To investigate the effects of budesonide on respiratory function and cellular immunity in children with acute bronchial asthma attack. Methods: A total of 130 children with acute bronchial asthma admitted to a hospital from March 2020 to March 2021 were selected and randomly divided into the conventional group(n=65, receiving conventional anti-asthma treatment) and the treatment group(n=65, receiving budesonide suspension nebulization inhalation treatment on the basis of conventional treatment). Respiratory function, serum inflammatory factor levels, symptom resolution time, T lymphocyte subsets, total clinical effective rate, and total incidence of adverse drug reactions were compared between the two groups before and after medication. Results: After treatment, forced expiratory volume in the first second(FEV1), forced expiratory flow at 75% of FVC capacity(FEF75), FEV1/forced vital capacity(FEV1/FVC), CD4+, and CD4+/CD8+ ratios increased in both groups, with higher values in the treatment group than in the conventional group(P<0.05). Serum levels of inflammatory factors [procalcitonin(PCT), high-sensitivity C-reactive protein(hs-CRP), interleukin-8(IL-8)]and CD8+ decreased in both groups, with lower levels in the treatment group(P<0.05). The resolution time of clinical symptoms(cough, wheezing, pulmonary rales, and dyspnea) in the treatment group was significantly shorter than that in the conventional group(P<0.05). The total clinical effective rate of the treatment group(92.31%) was higher than that of the conventional group(78.46%, χ2=4.993, P=0.025). No statistically significant difference in the total incidence of adverse drug reactions was observed between the two groups(χ2=0.769, P=0.380). Conclusion: Budesonide suspension nebulization inhalation can significantly relieve clinical symptoms, and improve respiratory function and immune function in children with acute bronchial asthma attack.
