【摘要】 目的：探讨复方羚角降压片联合常规疗法治疗难治性高血压的临床疗效及对患者血管弹性水平的影响。方法：选取2022年6月1日～2023年6月1日期间于某院心血管内科就诊的102例难治性高血压患者作为研究对象，采用随机数字表法分为对照组和观察组，每组51例。两组患者均严格控制生活及饮食方式，对照组患者给予硝苯地平缓释片（Ⅱ）、替米沙坦片、氢氯噻嗪片治疗，观察组患者在对照组治疗基础上加用复方羚角降压片，两组均治疗8周。比较两组收缩压和舒张压降低值、动脉脉搏波传导速度[颈-股动脉脉搏波传导速度（cfPWV）和臂踝脉搏波传导速度（baPWV）]、血清胱抑素C(Cys-C)、超敏C反应蛋白（hs-CRP）、一氧化氮（NO）水平、临床疗效及不良反应发生情况。结果：治疗后，观察组患者收缩压和舒张压降低值均高于对照组（P<0.05）。两组患者cfPWV和baPWV均降低（P<0.05），且观察组低于对照组（P<0.05）；血清Cys-C和hs-CRP水平均降低（P<0.05），且观察组低于对照组（P<0.05）；血清NO水平均升高（P<0.05），且观察组高于对照组（P<0.05）。观察组患者治疗总有效率（94.12%）高于对照组（78.43%,P=0.021）。两组患者不良反应总发生率比较无统计学差异（P=0.818）。结论：复方羚角降压片联合常规疗法治疗难治性高血压效果较佳，可有效改善患者血管弹性水平，且不会增加不良反应的发生风险。更多还原

【Abstract】 Objective: To investigate the clinical efficacy of compound Lingjiao hypotensive tablets combined with conventional therapy in the treatment of refractory hypertension and its effect on vascular elasticity. Methods: A total of 102 patients with refractory hypertension treated in the department of cardiovascular medicine of a hospital from June 1, 2022 to June 1, 2023 were selected and assigned to the control group and observation group by random number table method, with 51 patients in each group. Lifestyle and diet were strictly controlled for patients in both groups.The control group was treated with nifedipine sustained-release tablets(Ⅱ), telmisartan tablets,hydrochlorothiazide tablets, and the observation group was treated with compound Lingjiao hypotensive tablets in addition to the treatment given in the control group. Both groups were treated for 8 weeks. The reduction in systolic and diastolic blood pressure, arterial pulse wave velocity [carotid-femoral pulse wave velocity(cfPWV) and brachial-ankle pulse wave velocity(baPWV)], serum Cystatin C(Cys-C), hypersensitive C-reaction protein(hs-CRP), nitric oxide(NO) level, clinical efficacy and adverse reactions were compared between the two groups.Results: After treatment, the reduction in systolic and diastolic blood pressure was higher in the observation group compared to the control group(P<0.05). The cfPWV and baPWV were decreased in both groups(P<0.05), with the observation group showing lower levels than the control group(P<0.05). The levels of serum Cys-C and hs-CRP were decreased in both groups(P<0.05), with the observation group showing lower levels than the control group(P<0.05). The serum NO levels were increased in both groups(P<0.05), with the observation group showing higher levels than the control group(P<0.05). The total effective rate in the observation group(94.12%) was higher than that in the control group(78.43%, P=0.021). There was no statistically significant difference in the total incidence of adverse reactions between the two groups(P=0.818). Conclusion: Compound Lingjiao hypotensive tablets combined with conventional therapy shows better clinical efficacy in the treatment of refractory hypertension, effectively improving the level of vascular elasticity, without increasing the risk of adverse reactions.更多还原
