【摘要】 慢性非特异性颈痛是一种病因复杂、病程迁延的常见肌肉骨骼系统疾病，严重影响患者的生活质量和社会功能。近年来，针刺因其简便、安全及疗效确切，在慢性非特异性颈痛治疗中逐渐受到关注。临床研究表明，针刺能够有效缓解颈部疼痛、改善颈椎活动度和肌肉功能，同时在调节患者焦虑、抑郁等负面情绪方面亦具有积极作用。不同针刺形式包括体针、电针、内热针、浮针及穴位埋线、揿针等，均显示出良好的疗效，但治疗参数、选穴方案及疗程设计尚缺乏统一规范。在作用机制方面，现代研究提示针刺可能通过多途径发挥镇痛和康复效应：调控炎症因子水平及相关信号通路，抑制炎性反应；干预氧化应激状态，提升机体抗氧化能力；调节中枢神经系统功能，改善导水管周围灰质、海马及额叶等相关脑区的结构与连接；增强本体感觉功能，改善颈椎稳定性与运动协调性。文章通过整理相关文献，以期为慢性非特异性颈痛的临床治疗提供更加科学和可靠的证据支持。更多还原

【Abstract】 Non-specific chronic neck pain(NCNP) is a common musculoskeletal disorder characterized by complex etiologies and a prolonged disease course, which significantly impairs patients’ quality of life and social functioning. In recent years, acupuncture has attracted increasing attention in the management of NCNP due to its simplicity, safety, and reliable efficacy. Clinical studies have demonstrated that acupuncture can effectively relieve neck pain, improve cervical spine mobility and muscle function, and alleviate negative emotions such as anxiety and depression. Various acupuncture modalities, including body acupuncture, electro-acupuncture, internal heat acupuncture, floating needling, acupoint catgut embedding therapy, and press-needle therapy, have shown favorable outcomes, however, treatment parameters, acupoint selection strategies, and course designs remain to be standardized. Mechanistically, contemporary research suggests that acupuncture exerts analgesic and rehabilitative effects through multiple pathways: modulating inflammatory mediators and related signaling pathways to suppress inflammation, attenuating oxidative stress while enhancing endogenous antioxidant capacity, regulating central nervous system activity and improving structural and functional connectivity of pain-related brain regions such as the periaqueductal gray, hippocampus, and frontal cortex, and enhancing proprioceptive function to restore cervical stability and motor coordination. This article aims to provide more scientific and reliable evidence to support the clinical application of acupuncture in the treatment of NCNP by collating relevant literature.更多还原
