【摘要】 随着现代生活方式的改变，高热量饮食、缺乏运动、熬夜等因素的积累造成代谢综合征患者越来越多。代谢综合征是以腹型肥胖为外在表现，以高血压、高血脂、高血糖、高尿酸等多种代谢物质紊乱为特征的复杂性病理状态。清阳、浊阴理论来源于《黄帝内经》，与人体精微物质代谢息息相关，由“清阳出上窍”“浊阴出下窍”可见机体清升浊降功能与窍牗的关系；“清阳发腠理”“清阳实四支”指出了清阳在温养四肢、腠理及御外等方面的作用；“浊阴走五脏”“浊阴归六腑”是对脏腑消化、吸收、封藏水谷精微过程的概括。膏浊致病是仝小林院士基于《黄帝内经》并结合临床经验对多种代谢物质紊乱疾病的认识。仝小林院士认为，精微物质过剩并堆积于体内可变生病理性膏浊。其中“膏”为过剩之体脂，“浊”衍化为糖浊、脂浊、尿酸浊等危害机体。文章结合历代医家对该理论的注解与运用，认为过食肥甘影响脾胃运化功能进而导致代谢综合征，提出“健脾胃、升清阳、消膏脂；补肝肾、化浊阴、排浊毒”的多途径治疗思路，用药方面结合《黄帝内经》中相关理论与仝小林院士对膏浊致病的认识，采用自拟经验方升清化浊汤，为临床诊治代谢综合征提供参考。更多还原

【Abstract】 With the change of modern lifestyle, the accumulation of factors such as high-calorie diet, lack of exercise, and staying up late has caused more and more people to suffer from metabolic syndrome. Metabolic syndrome is a complex pathological state characterized by the external manifestation of abdominal obesity, and internal manifestation of various metabolic substance disorders such as hypertension, hyperlipidemia, hyperglycemia, and uric acid. The theory of “clear Yang( 阳) and turbid Yin( 阴)” originates from Huangdi Neijing(《黄帝内经》), and is closely related to the metabolism of human nutrient substances. The theories of “clear Yang comes out of the upper orifices” and “turbid Yin comes out of the lower orifices” points out the relationship between the functions of clearing up clear Yang and lowering turbid Yin of the human body and nine orifices. The theories of “clear Yang spreading into the body interstices” and “clear Yang nourishes the limbs” refer to the function of clear Yang in warming the limbs, and skin and orifices, and resisting external pathogenic factors. The theories of “turbid Yin runs through five-zang organs” and “turbid Yin tends to six-fu organs” refer to a summary of the process of digestion, absorption, and sealing of essence of water and grain. The theory of pathogenic high fatty diet is Academician TONG Xiaolin’s understanding of a variety of metabolic disorders diseases based on the relevant theory in Huangdi Neijing and the clinical experience. Academician TONG Xiaolin believes that the accumulation of excess subtle substances in the body will turn into pathological turbidity for a long time, of which “overmuch fine grain” is excess body fat, and “turbidity” derives into sugar turbidity, fat turbidity, uric acid turbidity and other pathological products that hazard the human body. Based on the commentary and application of the theory by doctors of the past dynasties, the author thinks that overeating fat diet can affect the function of spleen and stomach and lead to metabolic syndrome, and puts forward the multi-channel treatment ideas of “strengthening spleen and stomach to rise clear Yang and eliminate overmuch fine grain, and tonifying liver and kidney to remove turbid Yin and expel other turbid poisons”. In terms of medication of the disease, combined with the relevant theory in Huangdi Neijing and Academician TONG Xiaolin’s understanding of the theory of pathogenic high fatty diet, the Shengqing Huazhuo decoction( 升清化浊汤), a self-made empirical prescription, is used to treat metabolic syndrome clinically, providing more reference for the clinical diagnosis and treatment of metabolic syndrome.更多还原
