导管反应在慢性胆汁淤积性肝病中的作用
The role of ductular reaction in chronic cholestatic liver diseases
【摘要】 胆汁淤积性肝病是由免疫、遗传、感染、结石和肿瘤等因素导致的胆汁形成、分泌和排泄障碍的肝胆管疾病,其特点在于以胆管细胞为病变的主要靶点,伴随慢性炎症、胆汁淤积、肝内外胆管损伤以及肝纤维化等病理表现,可逐渐发展为肝硬化,甚至肝癌。由于缺少有效的药物,大多数患者最终需要肝移植。虽然目前对于慢性胆汁淤积性肝病有一定的认识,但其病因和发病机制仍不明确。导管反应是一种导管表型的反应,是慢性胆汁淤积性肝病的主要病理变化。本文聚焦于导管反应,阐述其在慢性胆汁淤积性肝病机制中的研究进展,并综述导管反应相关靶点、药物以及新治疗策略等,以期为慢性胆汁淤积性肝病治疗提供新的思路。
【Abstract】 Cholestatic liver disease is a disease of the liver and bile ducts caused by factors such as immunity, genetics, infection, stones and tumors, which results in bile formation, secretion and excretion disorders. It is characterized by cholangiocyte as the main target of the lesion and is accompanied by pathological manifestations such as chronic inflammation, cholestasis, intra-and extra-hepatic bile duct damage, and hepatic fibrosis, which can gradually develop into cirrhosis or even liver cancer. Due to the lack of effective medical therapies, most patients eventually require liver transplantation. Although there is some understanding of chronic cholestatic liver disease, its etiology and pathogenesis remain unclear. The ductular reaction, a reaction of ductular phenotype, is a major pathological change in chronic cholestatic liver disease. This article focuses on ductular reaction, elaborates its research advances in the mechanism of chronic cholestatic liver disease, and reviews the relevant targets, drugs, and new therapeutic strategies of ductular reaction to provide new ideas for the treatment of chronic cholestatic liver disease.
