新质生产力视角下重症急诊非技术技能培训现状分析
Research on the Status of Non-Technical Skills Training on Emergency and Critical Intensive Care Unit from the Perspective of New Quality Productivity
【摘要】 目的:探索医院高质量发展中非技术技能培训的重要性,分析当前非技术技能培训的现状,为提高新质生产力提供建议。方法:采用半结构式访谈法对珠三角地区各层级医院的急诊和重症医师进行访谈,并应用六步主题分析法进行编码和分析重症急诊非技术技能培训现状。结果:根据15位受访者的访谈记录,提炼出4个主题:1)非技术技能培训能通过提升管理效率、增强医患沟通、降低运营成本来促进医院高质量发展。2)重症急诊医师的非技术技能培训需求集中在领导力激励、病情评估决策、团队协作沟通、跨学科协作、患者管理沟通、应急危机管理。3)目前重症急诊的非技术技能培训不够成熟,存在培训标准不统一、时间与资源限制、定义和认知不清的情况。4)急危重症医师非技术技能可通过营造文化氛围、强化反馈与辅导、开展模拟演练及加强制度建设等路径得以提升。结论:非技术技能培训能够从新质生产力的劳动力层面推动医院高质量发展。尽管当前医院对非技术技能的认知和重视程度正在逐步提升,但重症急诊的非技术技能培训体系方面尚存优化空间,重症急诊医师的非技术技能水平亟待提升,且该群体在非技术技能培训领域的内驱力有待进一步深化。
【Abstract】 Objective To explore the importance of non-technical skills training in hospital quality development, analyze the current status of non-technical skills training, and provide suggestions for improving new quality productivity. Methods Semi-structured interviews were conducted with emergency and critical care physicians at all levels of hospitals in the Pearl River Delta region, and a six-step thematic analysis was applied to code and analyze the current status of non-technical skills training in critical care emergency medicine. Results Based on the interview of 15 interviewees, four themes were extracted. Non-technical skills training can promote the hospital high-quality development by improving management efficiency, enhancing doctor-patient communication, and reducing operational costs. The non-technical skills training needs of critical care emergency physicians were focused on leadership motivation, decision-making for condition assessment, collaborative teamwork and communication, interdisciplinary collaboration, patient management and communication, and emergency crisis management. The current non-technical skills training in critical care emergency medicine was not mature enough, with inconsistent training standards, time and resource constraints, and unclear definitions and perceptions. The non-technical skills of critical care physicians can be improved through the creation of a cultural environment, enhanced feedback and coaching, simulation exercises, and institutional strengthening. Conclusion Non-technical skills training can contribute to the overall quality development of hospitals at the level of a newly productive workforce. Although the awareness and value of non-technical skills in hospitals is gradually increasing, there is still room for optimization of the non-technical skills training system in critical care. The non-technical skills of critical care physicians need to be improved, and the internal drive of this group in the field of non-technical skills training needs to be further deepened.
