脑电图在脓毒症相关脑病中的诊断及预后评估的研究进展

【摘要】 脓毒症相关脑病(Sepsis-associated encephalopathy, SAE)是一种基于脓毒症排除中枢神经系统感染后发生的脑功能障碍综合征,发病率高达60%~80%(根据重症监护病房流行病学数据),但缺乏特异性诊断标准。传统临床评估(如意识状态评分)存在主观性,影像学(如核磁共振成像/计算机断层扫描)对早期脑功能损伤敏感性不足,导致诊断延迟与治疗滞后。脑电图(EEG)作为无创、动态监测脑电活动的工具,可捕捉脓毒症引发的神经元代谢异常、炎症介质释放及血流动力学紊乱导致的脑电特征改变。本文主要综述EEG对脓毒症及其相关脑病的诊断价值以及对患者不良结局的预测效能。

【Abstract】 Sepsis-associated encephalopathy(SAE) is a syndrome characterized by brain dysfunction that manifests after excluding central nervous system infections in the context of sepsis.The incidence of SAE is reported to be between 60% and 80%,according to epidemiological data from intensive care units.However, there is a notable absence of specific diagnostic criteria for this condition.Traditional clinical assessments, such as consciousness status scores, are inherently subjective, and imaging modalities like MRI and CT lack the sensitivity required to detect early brain function impairment, often leading to delays in diagnosis and treatment.In contrast, electroencephalography(EEG) serves as a non-invasive and dynamic tool for monitoring cerebral electrical activity.It is capable of detecting alterations in brain electrical characteristics resulting from sepsis-induced neuronal metabolic abnormalities, the release of inflammatory mediators, and hemodynamic disturbances.This review primarily focuses on elucidating the diagnostic utility of EEG in the context of sepsis and related neurological disorders, as well as its prognostic efficacy in predicting adverse outcomes in patients.
