2010-2021年山东省发热伴血小板减少综合征流行特征及影响因素
Epidemiological characteristics and influencing factors of fever with thrombocytopenia syndrome in Shandong Province from 2010 to 2021
【摘要】 目的 探讨山东省发热伴血小板减少综合征(SFTS)的流行特征、时空分布及影响其传播和发展的关键因素,为山东省SFTS防控提供科学依据。方法 基于2010-2021年山东省SFTS报告病例,采用全局空间自相关、局部空间自相关分析山东省SFTS的时空分布特征,识别SFTS聚集区,运用地理探测器探究自然环境因素与社会经济因素及其交互作用对SFTS流行的影响。结果 2010-2021年山东省累计报告SFTS病例5 703例,年均发病率0.47/10万,季节性高峰集中在每年的5-10月,重点人群为40-80岁的农民,疫情分布具有空间聚集性并逐年增强,主要集中在山东省中部地区和胶东半岛地区;气温、降水量、风速、海拔、坡度、NDVI及人口密度对SFTS的影响均有统计学意义,年均温度对SFTS发病率影响最大(q=0.365),其次是坡度(q=0.329);任意两个影响因素之间的交互作用均能增加SFTS的发病风险,坡度与风速之间的交互作用产生的影响力最大。结论 山东省SFTS发病人数呈增长趋势,存在空间聚集性,应针对SFTS高风险地区,实施强化监测与评估机制,制定并落实综合防控策略,以有效降低SFTS传播风险并保护公众健康。
【Abstract】 Objective To explore the epidemic characteristics, spatiotemporal distribution and key factors influencing the transmission and development of severe fever with thrombocytopenia syndrome(SFTS) in Shandong Province, providing a scientific basis for the prevention and control of SFTS in Shandong Province. Methods Based on reported SFTS cases in Shandong Province from 2010 to 2021,global local spatial autocorrelation analysis were conducted to identify the spatial and temporal distribution characteristics and clustering areas of SFTS in Shandong Province, and the influence of natural environmental factors, social and economic factors and their interaction on the prevalence of SFTS was explored by using geographic detectors. Results From 2010 to 2021,a cumulative total of 5 703 cases of SFTS were reported in Shandong Province, with an average annual incidence rate of 0.47/100 000. The seasonal peak was concentrated from May to October each year, and the key population was farmers aged 40 to 80. The epidemic distribution showed spatial aggregation and gradually strengthened, mainly concentrated in the central region of Shandong Province and the Jiaodong Peninsula region. Temperature, precipitation, wind speed, altitude, slope, NDVI and population density all had statistically significant influences on SFTS. Among them, the annual average temperature had the greatest impact on the incidence rate of SFTS(q=0.365),followed by the slope(q=0.329). The interaction between any two influencing factors could increase the risk of SFTS occurrence, and the interaction between the slope and wind speed had the greatest influence. Conclusion The number of SFTS cases in Shandong Province is showing an increasing trend, with spatial clustering. Relevant departments should implement an enhanced monitoring and assessment mechanism for areas at high risk of SFTS,formulate and implement comprehensive prevention and control strategies, in order to effectively reduce the transmission risk of SFTS and protect public health.
