剪切波弹性成像技术对超微针刀治疗肩袖损伤的疗效评价
Evaluation of the efficacy of shear wave elastography in the treatment of rotator cuff injuries with ultra-micro needle knife
【摘要】 目的 探讨剪切波弹性成像(SWE)在评估超微针刀治疗肩袖损伤疗效中的价值。方法 选取2022年8月~2024年2月于我院就诊的25例(31个肩关节)肩袖损伤患者,均接受超微针刀治疗。分别于治疗前、治疗2周后及治疗4周后对患者进行二维超声及SWE检查,测量冈上肌肌腱的定量指标。同时,结合视觉模拟疼痛评分(VAS)、肩关节外展活动度测量及肩关节前屈活动度测量,综合评估超微针刀治疗的疗效。结果 31个肩关节的冈上肌肌腱在超微针刀治疗前、治疗2周后及治疗4周后,杨氏模量的平均值、最大值、最小值均呈下降趋势,与治疗前相比差异具有统计学意义(P<0.05);剪切波速度值(SWV)在治疗后亦明显降低,与治疗前相比差异有统计学意义(P<0.05)。此外,31个肩关节的VAS评分在治疗后显著下降,前屈活动度和外展活动度显著增加,与治疗前相比差异有统计学意义(P<0.05)。冈上肌肌腱的杨氏模量平均值与肩关节VAS评分呈正相关(r=0.527,P<0.001),SWV与VAS评分呈正相关(r=0.572,P<0.001)。结论 SWE技术在评估肩袖损伤超微针刀治疗疗效方面具有一定的价值,值得在临床推广和进一步研究。
【Abstract】 Objective To explore the value of shear wave elastography(SWE) in evaluating the efficacy of ultra-micro needle knife therapy for rotator cuff injuries. Methods A total of 25 patients(31 shoulder joints) with rotator cuff injuries who were treated in our hospital from Aug 2022 to Feb 2024 were selected, and all received ultra-micro needle knife therapy. Twodimensional ultrasound and SWE examinations were performed on the patients before treatment, 2 weeks after treatment, and 4weeks after treatment to measure the quantitative indicators of the supraspinatus tendon. Meanwhile, the efficacy of ultra-micro needle knife therapy was comprehensively evaluated by combining visual analog scale(VAS) pain score, active assisted range of motion of shoulder abduction measurement, and active assisted range of motion of shoulder flexion measurement. Results For the supraspinatus tendons of the 31 shoulders joints, the mean, maximum, and minimum values of Young′s modulus showed a decreasing trend before ultra-micro needle knife treatment, 2 weeks after treatment, and 4 weeks after treatment, with statistically significant differences compared with those before treatment(P<0.05). The shear wave velocity(SWV) also significantly decreased after treatment, with a statistically significant difference compared with that before treatment(P<0.05). In addition, the VAS scores of the 31 shoulder joints significantly decreased after treatment, while the forward flexion range of motion and abduction range of motion significantly increased, with statistically significant differences compared with those before treatment(P<0.05). The mean Young′s modulus of the supraspinatus tendon was positively correlated with the shoulder VAS score(r=0.527, P<0.001), and the SWV was positively correlated with the VAS score(r=0.572, P<0.001).Conclusion SWE technology has certain value in evaluating the efficacy of ultra-micro needle knife therapy for rotator cuff injuries, and is worthy of clinical promotion and further research.
