【摘要】 目的 研究和分析初发疑似痛风性关节炎发作患者双能CT表现与血尿酸水平之间的相关性。方法 以2022年1月—2024年12月在本院进行诊治的疑似痛风性关节炎患者106例作为研究对象，为患者提供双能CT检查及血尿酸检查，分析痛风性关节炎检出情况、尿酸盐结晶体积、分布部位及范围，比较痛风性关节炎与非痛风性关节炎患者血尿酸水平，针对痛风性关节炎患者双能CT表现、血尿酸水平及相关性进行分析。结果 58例患者明确诊断为痛风性关节炎，确诊率为54.72%。痛风性关节炎患者与非痛风性关节炎患者踝关节、手关节、腕关节尿酸盐结晶检出率差异无统计学意义(P>0.05)。痛风性关节炎患者与非痛风性关节炎患者髌骨及膝关节、足骨及足关节尿酸盐结晶检出率差异有统计学意义(P<0.05)。痛风性关节炎患者血尿酸水平明显高于非痛风性关节炎患者，两组患者血尿酸水平差异有统计学意义(P<0.05)。不同双能CT表现者的血尿酸水平差异具有统计学意义(P<0.05)。骨侵蚀及痛风石与血尿酸水平呈正相关(P<0.05)，关节腔积液、软组织肿胀、滑膜增生与血尿酸水平无相关性(P>0.05)。结论 初发疑似痛风性关节炎发作患者骨侵蚀及痛风石等双能CT表现与血尿酸水平呈正相关，对患者实施双能CT检查有助于辅助临床明确诊断。更多还原

【Abstract】 Objective To study and analyze the correlation between dual-energy CT findings and serum uric acid levels in patients with suspected gouty arthritis.Methods Take 106 patients with suspected gouty arthritis who were diagnosed and treated in our hospital from January 2022 to December 2024 as the research object,provide dual-energy CT examination and blood uric acid examination for patients,analyze the detection situation of gouty arthritis,the volume of urate crystals,the distribution location and range,compare the blood uric acid levels of gouty arthritis patients and non-gouty arthritis patients,and analyze the dual-energy CT manifestations,blood uric acid levels and correlation of gouty arthritis patients.Results The diagnosis rate of gouty arthritis was 54.72%.There was no significant difference in the detection rate of urate crystals in ankle joint,hand joint and wrist joint between patients with gouty arthritis and patients without gouty arthritis (P>0.05),but there was significant difference in the detection rate of urate crystals in patella,knee joint,foot bone and foot joint between the two groups (P<0.05).The serum uric acid level of patients with gouty arthritis was significantly higher than that of patients without gouty arthritis,and the difference between the two groups was statistically significant (P<0.05).There were significant differences in serum uric acid levels between different dual-energy CT manifestations (P<0.05).Bone erosion and tophus were positively correlated with serum uric acid level (P<0.05),while joint cavity effusion,soft tissue swelling and synovial hyperplasia were not correlated with serum uric acid level (P>0.05).Conclusion Dual-energy CT findings such as bone erosion and tophi in patients with initial suspected gouty arthritis are related to serum uric acid level,and dual-energy CT examination is helpful to assist clinical diagnosis.更多还原
