黄帝内经督脉贯脊理论科学依据的初步研究

【摘要】 目的:初步探讨《黄帝内经》“督脉贯脊”理论的早期人类医学科学依据。方法:考证《黄帝内经》等古籍中督脉贯脊理论相关记载;采用体感诱发电位(SEP)比较分析24只SD大鼠(正常对照组、切断督脉组和切断脊髓组),针刺督脉命门穴“得气”感觉传导路线的电生理客观指标;采用神经组织透明三维成像方法,比较分析2只C57转基因小鼠脊柱区域督脉穴位的脊髓和神经的解剖结构。结果:古籍“督脉贯脊”记载的是督脉在椎管中通过,“刺脊中间髓,为伛”记载的是针刺损伤脊髓。体感诱发电位结果:切断督脉组与正常对照组间差异无统计学意义(P>0.05);切断脊髓组与正常对照组间差异有统计学意义(P<0.01);切断脊髓组与切断督脉组间差异有统计学意义(P<0.01);神经组织透明三维成像显示,督脉穴区的神经末梢分布密集(9条/mm~2),假穴区分布稀疏(3条/mm~2)。结论:针刺大鼠脊柱区域督脉命门穴“得气”感觉传导通路的解剖学和电生理学基础是脊髓和神经。

【Abstract】 Objective:To explore the scientific basis of early human medicine related to the Governor Vessel(GV)in the Huangdi Neijing.Methods:Ancient texts such as the Huangdi Neijing were examined;Somatosensory evoked potentials(SEP)were used to comparatively analyze 24 SD rats(divided into 3 groups:control group,Governor Vessel transection group,and spinal cord transection group),focusing on electrophysiological objective indicators of the“Deqi”sensation during acupuncture at the Governor Vessel’s Mingmen acupoint;Comparative analysis of transparent three-dimensional imaging of Governor Vessel tissue regions was conducted in 2 C57 transgenic mice to study spinal nerve distribution in Governor Vessel acupoints.Results:In the Suwen Gukong Lun,the Governor Vessel was referred to as passing through the vertebral canal,and in the Suwen Cijin Lun,acupuncture-induced spinal cord injury leading to sensory and motor dysfunction in the back muscles and lower limbs was described.SEP results:No significant difference was found between the Governor Vessel transection group and the control group(P>0.05);a statistically significant difference was observed between the spinal cord transection group and the control group(P<0.01);a statistically significant difference was also noted between the spinal cord transection group and the Governor Vessel transection group(P<0.01);Transparent three-dimensional imaging of neural tissue revealed dense nerve endings in the Governor Vessel acupoint region(9 fibers/mm~2)compared to sparse distribution in sham acupoint regions(3 fibers/mm~2).Conclusion:The anatomical and electrophysiological basis for the“Deqi”sensation conduction pathway during acupuncture at the Mingmen acupoint of the Governor Vessel in the rat spinal region lies in the spinal cord and nerves.
