北京市某区1起成人百日咳聚集性感染调查

【摘要】 对北京市某区1起成人百日咳聚集性感染病例开展流行病学调查,采集患者血、鼻咽拭子样本进行血清百日咳杆菌毒素IgG抗体(pertussis toxin IgG,PT-IgG)检测、百日咳杆菌实时荧光聚合酶链反应(polymerase chain reaction,PCR)检测、百日咳杆菌培养,并对分离株进行全基因测序和抗生素最小抑菌浓度(minimal inhibition concentration,MIC)分析。结果显示,本起百日咳聚集性感染涉及4例成人实验室诊断病例,表现为“成人-成人”的传播模式。2例患者百日咳杆菌培养阳性,分离菌株均为ST2型,百日咳杆菌毒素抗原分型为ptxP3型,2株菌株均对红霉素、阿奇霉素、克林霉素和磺胺甲噁唑表现为“绝对耐药”,MIC分别为>256、>256、>256、>1 024μg/ml。对实验室结果的有效解读可辅助判断聚集性或暴发疫情中病例发病时间和传播顺序,需持续加强区域内百日咳流行株药物敏感性监测,提升疾病控制和临床治疗的精准度。

【Abstract】 An epidemiological investigation was conducted on the adult pertussis outbreak in a certain District,Beijing,collecting blood and nasopharyngeal swab samples from patients for serological testing of pertussis toxin IgG antibody(PT-IgG),real-time fluorescence polymerase chain reaction(PCR) of Bordetella pertussis and bacterial culture.Isolated strains underwent whole-genome sequencing and minimum inhibitory concentration(MIC)analysis for antibiotics.The outbreak involved four lab oratory-confirmed adult cases,exhibiting a "person-toperson" transmission pattern.Two patients tested positive for Bordetella pertussis culture,with both isolates identified as ST2 type and categorized as ptxP3 antigen type.Both strains exhibited "absolute resistance" to erythromycin,azithromycin,clindamycin,and sulfamethoxazole,with MIC values exceeding 256,256,256,and1 024 μg/ml,respectively.Effective interpretation of laboratory results can aid in determining the onset and transmission sequence of cases during outbreaks.Continuous monitoring of drug sensitivity for prevalent pertussis strains in the region is necessary to enhance the precision of disease control and clinical treatment.
