基于肺部超声引导的中频脉冲电治疗联合呼吸训练在机械通气患者早期肺康复中的应用效果
Application effect of mid-frequency pulse electrotherapy combined with breathing training based on lung ultrasound guidance to the early pulmonary rehabilitation of patients with mechanical ventilation
【摘要】 目的 探讨基于肺部超声引导的中频脉冲电治疗联合呼吸训练在机械通气患者早期肺康复中的应用效果。方法 选取2022年6月至2024年6月九江市第一人民医院ICU收治的机械通气治疗的91例患者作为研究对象,采用随机数字表法将其分为对照组(45例)和观察组(46例)。对照组采用常规康复干预,观察组采用基于肺部超声引导的中频脉冲电治疗联合呼吸训练方案。两组均持续干预至患者出院。比较两组患者的肺部超声评分、肺部感染、氧合指数、膈肌厚度、生命体征、机械通气时间等指标变化。结果 观察组干预后肺部超声评分、肺部感染评分均低于对照组,氧合指数高于对照组,差异有统计学意义(P<0.05);观察组干预后呼气末膈肌厚度、吸气末膈肌厚度均大于对照组,差异有统计学意义(P<0.05);观察组干预后心率、收缩压以及呼吸频率均低于对照组,差异有统计学意义(P<0.05);观察组机械通气时间、入住ICU时间均短于对照组,差异有统计学意义(P<0.05)。结论 基于肺部超声引导的中频脉冲电治疗联合呼吸训练可促进患者机械通气患者肺功能康复,有效减少肺部感染发生,改善膈肌厚度,利于维持生命体征稳定,从而缩短机械通气时间。
【Abstract】 Objective To investigate the intervention effect of lung ultrasound-guided medium frequency pulse electrotherapy combined with breathing training in patients with mechanical ventilation. Methods A total of 91 patients treated with mechanical ventilation in ICU, Jiujiang NO.1 People’s Hospital from June 2022 to June 2024 were selected as the research objects, and they were divided into control group(45 patients) and observation group(46 patients) by random number table method. The control group received conventional rehabilitation intervention, while the observation group adopted lung ultrasound-guided intermediate frequency pulse electrotherapy combined with respiratory training regimen. The pulmonary ultrasound score, pulmonary infection, oxygenation index, diaphragm thickness, vital signs and mechanical ventilation time were compared between the two groups. Results After intervention, ultrasound scores and pulmonary infection scores in observation group were lower than those in control group, oxygenation index was higher than that in control group, and the differences were statistically significant(P<0.05). The diaphragm thickness at the end of expiratory and inspiratory in the observation group was higher than that in the control group, and the difference was statistically significant(P<0.05). After intervention, the heart rate, systolic blood pressure and respiratory rate of observation group were lower than those of control group, and the differences were statistically significant(P<0.05). The mechanical ventilation time and ICU stay time of observation group were shorter than those of control group, and the differences were statistically significant(P<0.05). Conclusion Medium frequency pulse electrotherapy combined with breathing training based on lung ultrasound guidance can promote the pulmonary function rehabilitation of patients with mechanical ventilation, effectively reduce the occurrence of lung infection, improve the thickness of diaphragm, help maintain the stability of vital signs, and shorten the time of mechanical ventilation.
