【摘要】 目的：分析新型部分髋关节置换术治疗股骨头坏死（ONFH）部分塌陷的短期疗效。方法：回顾性分析2021年10月至2023年7月于首都医科大学附属北京积水潭医院接受新型部分髋关节置换术的19例（21髋）患者的临床资料。记录患者术前及末次随访时髋关节疼痛视觉模拟评分（VAS）及髋关节Harris评分（HHS），并进行比较；同时观察并发症发生情况。定义患者接受全髋关节置换术（THA）为保关节失败，绘制Kaplan-Meier生存曲线计算保关节生存率。结果：本研究19例（21髋）患者随访12～31个月，平均（19.8±5.3）个月。6髋在术后平均（19.8±3.8）个月时进行THA。末次随访时患者髋关节VAS评分和HHS评分均较术前明显改善（P均<0.001）。术前股骨头坏死面积大的患者保关节失败概率更高。保关节失败的主要原因是ONFH进展所致假体失效。术后部分患者出现异位骨化和股骨头颈部骨性增生，无其他术后并发症发生。结论：新型部分髋关节置换术为ONFH部分塌陷的保关节治疗提供了新的选择。更多还原

【Abstract】 Objective: To evaluate the short-term outcomes of a novel partial hip arthroplasty technique for the treatment of partial femoral head collapse in osteonecrosis of femoral head. Methods: A retrospective analysis was conducted on 19 patients(21 hips) who underwent the novel partial hip arthroplasty at Beijing Jishuitan Hospital, Capital Medical University, between October 2021 and July 2023. Clinical data were collected, including Visual Analog Scale(VAS) scores for hip pain and Harris hip scores(HHS) preoperatively and at the final follow-up. These scores were compared to assess functional improvement. Postoperative complications were also recorded. Conversion to total hip arthroplasty(THA) was defined as joint preservation failure, and Kaplan-Meier survival analysis was performed to estimate the joint-preserving procedure survival rate. Results: All 19 patients(21 hips) were followed for 12 to 31months, with a mean duration of 19.8±5.3 months. Six hips(6/21) underwent conversion to THA at a mean of 19.8±3.8 months postoperatively. Both VAS and HHS scores showed significant improvement at the final follow-up compared to preoperative values(both P<0.001). Patients with larger necrotic areas preoperatively had a higher risk of joint-preserving failure. The primary reason for failure was prosthesis dysfunction due to the progression of ONFH. Some patients developed heterotopic ossification or bony overgrowth at the femoral head-neck junction, and no other major complications were observed. Conclusions: The novel partial hip arthroplasty technique provides a new joint-preserving surgical option for ONFH patients with partial femoral head collapse.更多还原
