2021-2024年云南省西南部5个县区18~45岁健康女性人乳头瘤病毒感染率和基因亚型分布
Prevalence and genotypes of human papillomavirus among healthy women 18-45 years of age during 2021-2024 in five counties/districts of southwestern Yunnan
【摘要】 目的 了解云南省成年女性人乳头瘤病毒(Human papillomavirus, HPV)感染率和基因亚型分布。方法 2021-2024年在云南省西南部5个县区招募18~45岁健康女性,采集子宫颈脱落细胞,采用聚合酶链反应(Polymerase chain reaction, PCR)-反向点杂交法检测各基因亚型HPV DNA,分析HPV感染率。结果 研究对象总体HPV感染率为15.45%(695/4 499),其中18~20岁、21~25岁、26~30岁、31~35岁、36~40岁、41~45岁分别为36.45%、21.26%、14.35%、12.83%、14.41%、12.79%(χ~2=67.57,P<0.001)。共检出17种高危型和6种低危型HPV基因亚型,在高危型中居前5位的HPV52、HPV16、HPV51、HPV58、HPV53感染率分别为2.89%、2.24%、1.91%、1.60%、1.36%,在低危型中居前3位的HPV81、HPV42、HPV43感染率分别为1.20%、1.02%、0.96%;单一、混合HPV感染率分别为11.76%、3.69%。结论 研究地区18~45岁健康女性总HPV感染水平较高且随年龄增长而下降,以高危型HPV感染为主。建议加强适龄女性子宫颈癌筛查和HPV疫苗接种。
【Abstract】 Objective To explore infection prevalence and genotype distribution of human papillomaviruses(HPV) among adult women in Yunnan province.Methods We recruited healthy women 18-45 years of age in five counties/districts of southwestern Yunnan during 2021-2024. We collected cervical exfoliated cells and tested for DNA of each HPV genotype using polymerase chain reaction(PCR)-reverse dot hybridization to determine the HPV infection prevalence.Results The overall HPV infection prevalence was 15.45%(695/4 499), with 36.45%, 21.26%, 14.35%, 12.83%, 14.41%, and 12.79% for 18-20-, 21-25-, 26-30-, 31-35-, 36-40-, and 41-45-year-olds, respectively(χ~2=67.57, P<0.001). A total of 17 high-risk and 6 low-risk HPV genotypes were detected. Among high-risk genotypes, the 5 highest infection rates were with HPV52, HPV16, HPV51, HPV58, and HPV53 at 2.89%, 2.24%, 1.91%, 1.60%, and 1.36%, respectively. Among low-risk genotypes, the 3 highest infection rates were with HPV81, HPV42, and HPV43 at 1.20%, 1.02%, and 0.96%. Single and multiple infection rates were 11.76% and 3.69%.Conclusions The prevalence of HPV infection was high among healthy women 18-45 years of age in the study area and decreased with increasing age. High-risk HPV genotypes were predominant. We should strengthen cervical cancer screening and HPV vaccination for age-eligible women.
