2024年河南省3例输入性D8基因型麻疹病例的病原学特征分析

【摘要】 目的 分析河南省2024年3例输入性麻疹病例麻疹病毒(Measles virus, MV)分离株病原学特征。方法 开展麻疹病例流行病学调查,采用酶联免疫吸附试验检测血清MV IgM抗体,采用荧光定量聚合酶链反应检测咽拭子MV核酸,对核酸阳性标本进行MV细胞培养。采用逆转录聚合酶链反应扩增MV分离株序列,构建与参考株序列的亲缘性关系树,分析氨基酸和核苷酸序列一致性以及氨基酸变异。结果 3例麻疹病例包括儿童1例、成人2例,从其他国家或中国其他省份旅居河南省;且MV IgM抗体、核酸检测均阳性,其中1例细胞培养阳性。3株MV分离株与D8基因型参考株MVi/Manchester.GBR/30.94/D8属于同一个分支,分别与D8基因型命名株MVs/Rudaki.TJK/49.21、MVs/Almaty.KAZ/10.23、MVs/Patan.IND/16.19的氨基酸和核苷酸序列一致性最高,分别为100%和99.8%、100%和100%、100%和100%。3株分离株序列与D8基因型参考株、中国疫苗株序列分别存在3~4个、16~19个氨基酸变异位点。结论 本研究输入性麻疹病例MV分离株均为D8基因型,且存在一定变异。需继续加强流行病学和病原学监测,及时识别和处置输入性麻疹疫情。

【Abstract】 Objective To analyze genetic sequences of measles virus(MV) isolates from three imported measles cases in Henan province in 2024.Methods We conducted an epidemiological investigation of measles cases, tested sera for MV IgM antibody using enzyme-linked immunosorbent assay, tested throat swabs for MV nucleic acid using fluorescence quantitative polymerase chain reaction, and cultured nucleic acid positive isolates. We amplified the sequences of MV isolates using reverse transcription polymerase chain reaction and constructed a phylogenetic tree compared with reference strains to determine amino acid and nucleotide sequence consistency and amino acid variation.Results The three measles cases included one child and two adults who traveled from other countries or another province of China to Henan province. All cases were positive for MV IgM antibody and nucleic acid; one case was able to be cell cultured. The three MV isolates belonged to the D8 genotype reference strain MVi/Manchester.GBR/30.94/D8, with high amino acid and nucleotide sequence consistencies of 100% and 99.8%, 100% and 100%, and 100% and 100%, compared with D8 genotype strains MVs/Rudaki.TJK/49.21, MVs/Almaty.KAZ/10.23, and MVs/Patan.IND/16.19, respectively. The three isolates had 3-4 amino acid differences from the D8 genotype reference strain and 16-19 amino acid differences from the Chinese vaccine strain.Conclusions The MV isolates from imported measles cases were D8 genotype with some variation. It is necessary to strengthen epidemiological and etiological monitoring for rapid identification and responses of imported measles cases.
