上海市松江区≥60岁老年人接种23价肺炎球菌多糖疫苗预防社区获得性肺炎保护效果的真实世界研究
Real-world effectiveness of 23-valent pneumococcal polysaccharide vaccine against community-acquired pneumonia among elderly people ≥60 years of age in Songjiang district of Shanghai
【摘要】 目的 评价≥60岁老年人接种23价肺炎球菌多糖疫苗(23-valent pneumococcal polysaccharide vaccine, PPV23)预防社区获得性肺炎(Community-acquired pneumonia, CAP)的保护效果(Vaccine effectiveness, VE)。方法 2016年4月-2017年9月在上海市松江区招募首次接种和未接种PPV23的≥60岁人群开展前瞻性队列研究,观察截至2019年底两组CAP发病密度。采用Cox比例风险回归模型分析两组CAP发病风险比(Hazard ratio, HR),估算PPV23预防CAP的VE。结果 PPV23接种组、未接种组研究对象在观察期间CAP发病密度分别为1.33/100人年(48/3 615.94)、1.99/100人年(349/17 557.31),两组CAP发病的HR为0.68(95%CI:0.49~0.96),PPV23预防CAP的VE为31.66%(95%CI:4.44%~51.13%);经调整年龄后的VE为29.94%(95%CI:1.61%~50.11%)。结论 60岁老年人接种PPV23对预防CAP发病具有显著保护效果;建议老年人及时接种PPV23。
【Abstract】 Objective To evaluate the vaccine effectiveness(VE) of 23-valent pneumococcal polysaccharide vaccine(PPV23) against community-acquired pneumonia(CAP) in elderly people ≥60 years of age.Methods We recruited ≥60-year-olds who had received one dose of PPV23 and ≥60-year-olds who had not received PPV23 during April 2016 to September 2017 in Songjiang district of Shanghai and conducted a prospective cohort study. We determined incidence densities of CAP in the two groups to estimate their hazard ratio(HR) and the PPV23 VE against CAP using a Cox proportional hazard regression model.Results CAP incidence densities were 1.33 per 100 person-years(48 cases in 3 615.94 person-years) in the PPV23 vaccinated group and 1.99 per 100 person-years(349 cases in 17 557.31 person-years) in the unvaccinated group during the observation period. The HR was 0.68(95% CI: 0.49 to 0.96) and the VE of PPV23 against CAP was 31.66%(95% CI: 4.44% to 51.13%). Age-adjusted VE was 29.94%(95% CI:1.61% to 50.11%).Conclusions PPV23 vaccination provided significant protection against CAP among ≥60-year-olds. We recommend that elderly individuals receive PPV23.
