计算机认知训练对PSCI患者认知功能重塑的影响及优化策略

【摘要】 本研究旨在探讨自适应认知训练对脑卒中后认知障碍(PSCI)患者认知功能重塑的影响。选取2023年10月至2024年10月岳阳市中心医院康复医学科68例脑卒中患者,随机分为试验组和对照组各34例。对照组接受常规治疗、康复及护理,试验组在此基础上进行每天15-30分钟的自适应认知训练。干预前后采用蒙特利尔认知评估量表(MoCA)和简易智能精神状态量表(MMSE)评估认知功能,并对试验组进行质性访谈。结果显示,58例患者完成研究,试验组和对照组在年龄、干预前认知功能评分上差异无统计学意义。干预后,两组MoCA和MMSE得分均提高,且试验组进步幅度显著高于对照组(P<0.001)。质性研究显示,82%患者认可训练效果,认为其提升了日常生活能力及情绪状态。自适应认知训练可有效改善PSCI患者认知功能,但训练中建议根据患者个体差异制定分级训练计划,动态调整训练难度以提升康复效果。

【Abstract】 This study aimed to explore the impact of adaptive cognitive training on the cognitive function remodeling of patients with post-stroke cognitive impairment (PSCI).A total of 68 stroke patients admitted to the Department of Rehabilitation Medicine of Yueyang Central Hospital from October 2023 to October 2024 were selected and randomly divided into an experimental group and a control group of 34 patients each.The control group received routine treatment,rehabilitation,and nursing care,while the experimental group received an additional 15~30 minutes of adaptive cognitive training per day.The Montreal Cognitive Assessment Scale (MoCA) and the Mini-Mental State Examination (MMSE) were used to evaluate cognitive function before and after the intervention,and a qualitative interview was conducted with the experimental group.The results showed that 58 patients completed the study.There was no significant difference in age and pre-intervention cognitive function scores between the experimental group and the control group.After the intervention,the scores of MoCA and MMSE in both groups increased,and the improvement range in the experimental group was significantly higher than that in the control group (P<0.001).Qualitative studies show that 82% of patients recognized the effectiveness of the training,believing it improved their activities of daily living and emotional state.Adaptive cognitive training can effectively enhance cognitive function in PSCI patients.However,during training,it is recommended to develop graded training programs based on individual differences and dynamically adjust difficulty levels to optimize rehabilitation outcomes.
