【摘要】 目的 观察复方三黄液湿敷治疗复杂胫骨平台骨折后血性水疱的临床疗效。方法 选取2023年6月至2024年12月本院收治的复杂胫骨平台骨折患者68例，随机数字表法分为试验组和对照组，每组34例。试验组采用复方三黄液纱布湿敷，对照组则予无菌盐水纱布外敷。比较两组患肢肿痛改善情况(水疱复张数目、皮纹复现时间、VAS评分)、小腿肿胀周径比及血清创伤和炎症指标。结果 治疗期间试验组水疱复张数、皮纹复现时间优于对照组，第7天试验组VAS评分低于对照组(P<0.05)；治疗第5、7天试验组小腿肿胀周径比低于对照组(P<0.05)，同时第7天复查CK-MM、CRP值显示试验组低于对照组(P<0.05)。结论 复杂胫骨平台骨折后肿胀严重，常伴血性水疱，局部湿敷复方三黄液，既能促进肿痛的缓解，减少血性水疱的复张，改善软组织炎症情况，又能缩短术前等待时间，促进患者围手术期快速康复。更多还原

【Abstract】 Objective To observe the curative effect of wet compress of Compound Sanhuang Liquid in the treatment of bloody blister after complex tibial plateau fractures. Methods A total of 68 patients with complex tibial plateau fractures admitted to our hospital from June 2023 to December 2024 were selected and divided into the trial group and the control group by the random number table method, with 34 cases in each group. The trial group was treated with wet compress of compound Sanhuang liquid gauze, while the control group was given external application of sterile saline gauze. The improvement of swelling and pain in the affected limbs(the number of blisters reopening, the time of dermatoglyphic reappearance, VAS score), the ratio of swelling circumference of the lower leg, and the serum trauma and inflammatory indicators were compared between the two groups. Results During the treatment period, the number of blister recurrence and the recurrence time of dermatoglyphics in the trial group were better than those in the control group. On the 7th day, the VAS score of the trial group was lower than that of the control group(P<0.05). During the treatment period, the circumferential ratio of calf swelling in both groups showed a stepwise decreasing trend. On the 5th and 7th days of treatment, the circumference ratio of calf swelling in the trial group was lower than that in the control group(P<0.05). Meanwhile, the reexamination of CK-MM and CRP values on the 7th day showed that the values in the trial group were lower than those in the control group(P<0.05). Conclusions Swelling is usually severe and bloody blisters often occur after complex tibial plateau fractures. The local wet compress of Compound Sanhuang Liquid can not only relieve the swelling, reduce the re-expansion of bloody blisters, and improve soft tissue inflammation, but also shorten preoperative waiting time, facilitate patients’ rapid recovery during the perioperative period.更多还原
