心率变异性、心率减速力与正常体重肥胖儿童青少年心血管代谢性疾病的关联研究
Association Study of HRV, DC and Cardiovascular and Metabolic Diseases in Normal Weight and Obese Children and Adolescents
【摘要】 目的 研究心率变异性(HRV)、心率减速力(DC)与正常体重肥胖儿童青少年心血管代谢性疾病的相关性。方法 回顾性分析2023年1月至2024年12月89例在福建省三明市第一医院接受体检的儿童青少年(6~17岁)临床资料,据体重指数(BMI)和体脂百分比(BFP)分为正常体重肥胖组(n=31)、正常体型组(n=28)和超重肥胖组(n=30),比较3组HRV指标、DC及心血管代谢性疾病风险评分(CRS)差异,采用Spearman相关性分析法进行相关性分析,采用Logistic回归分析法进行CRS的危险因素分析。结果 3组间正常窦性N-N间期标准差(SDNN)、DC、CRS比较差异有统计学意义(P<0.05);Spearman相关性分析显示,BMI、BFP与CRS呈正相关,SDNN、DC与CRS呈负相关(P<0.05);Logistic回归分析显示,BFP是评估CRS的独立危险因素,DC是评估CRS的独立保护因素(P<0.05)。结论 正常体重肥胖儿童青少年存在HRV指标及DC的异常改变,且CRS显著升高。BMI、BFP与CRS呈正相关,SDNN、DC与CRS呈负相关。BFP是评估儿童青少年心血管代谢性疾病风险的独立危险因素,DC是评估儿童青少年心血管代谢性疾病风险的独立保护因素。
【Abstract】 Objective To investigate the correlation between heart rate variability(HRV), heart rate deceleration(DC), and cardiovascular and metabolic diseases in children and adolescents with normal weight obesity. Methods A retrospective analysis was conducted on the clinical data of 89 children and adolescents(aged 6-17) who underwent physical examinations at our hospital from January 2023 to December 2024. They were divided into three groups based on body mass index(BMI) and body fat percentage(BFP): normal weight obesity group(n=31), normal body shape group(n=28), and overweight obesity group(n=30). Differences in HRV indicators, DC, and cardiovascular and metabolic disease risk score(CRS) were compared among the three groups. Spearman correlation analysis was used for correlation analysis, and Logistic regression analysis was used for risk factor analysis of CRS. Results Significant differences were observed in the standard deviation of normal sinus N-N intervals(SDNN), DC, and CRS among the three groups(P<0.05). Spearman correlation analysis showed that BMI and BFP were positively correlated with CRS, while SDNN and DC were negatively correlated with CRS(P<0.05). Logistic regression analysis showed that BFP was an independent risk factor for evaluating CRS, and DC was an independent protective factor for evaluating CRS(P<0.05). Conclusions Children and adolescents with normal weight obesity exhibit abnormal changes in HRV indicators and DC, accompanied by significantly elevated CRS. BMI and BFP are positively correlated with CRS, while SDNN and DC are negatively correlated with CRS. BFP is an independent risk factor for assessing the risk of cardiovascular metabolic diseases in children and adolescents, while DC is an independent protective factor for assessing the risk of cardiovascular metabolic diseases in children and adolescents.
