24小时尿电解质在慢性肾病中的变化及其参考区间的建立

【摘要】 目的 分析24 h尿电解质在慢性肾病中的变化,初步评估其对慢性肾病的诊断价值,并制定本地区健康人群24 h尿钾、钠、钙、磷和镁的参考区间。方法 收集2020年7月至2021年3月在南阳市中心医院住院的140例慢性肾病患者的24 h尿标本,同时收集本地区166名健康人员的24 h尿标本,采用罗氏Cobas8000全自动生化分析仪测定钾、钠、钙、磷和镁,计算出24 h含量,比较慢性肾病组与健康人组之间的差异,根据健康人群制定这些指标的参考区间并进行验证。结果 慢性肾病组24 h尿磷、镁均高于健康人组,24 h尿钙低于健康人组,差异均有统计学意义(P<0.05),两组尿钾、钠比较,差异无统计学意义(P > 0.05);166名健康人员的24 h尿钾、钠、钙、磷和镁水平在性别上差异无统计学意义(P > 0.05),在不同年龄组差异也无统计学意义(P > 0.05),按照95%置信区间制定这些指标的正常参考值范围分别为16.70~80.70 mmol/d,73.10~261.80 mmol/d,1.05~8.11 mmol/d,5.22~27.44 mmol/d,1.27~5.23 mmol/d。结论 24 h尿钾、钠、钙、磷水平变化在慢性肾病中有适当的筛查价值,可进一步研究其对慢性肾病进展的影响,尿镁水平是否有差异还需进一步参考慢性肾病不同分期再进一步确认;本地区健康人群24 h尿钾、钠、钙、磷和镁在性别和年龄上差异无统计学意义,本研究成功建立了中原某地区健康人群24 h钾、钠、钙、磷和镁的参考区间。

【Abstract】 Objective To analyze the changes of 24-hour urinary electrolyte in chronic kidney disease, preliminally evaluate its diagnostic value for chronic kidney disease, and formulate the reference interval of 24-hour urinary potassium, sodium, calcium, phosphorus and magnesium in local healthy population. Methods The 24-hour urine samples of 140 patients with chronic kidney disease hospitalized in Nanyang Central Hospital from July 2020 to March 2021 were collected, and the 24-hour urine samples of 166 healthy people in this region were collected at the same time. The contents of potassium, sodium, calcium, phosphorus and magnesium were measured by Roche Cobas8000 automatic biochemical analyzer, and the 24-hour contents were calculated. The differences between the chronic kidney disease group and the healthy group were compared. Reference intervals were developed and validated against healthy populations. Results The 24-hour urinary phosphorus and magnesium levels in the chronic kidney disease group were higher than those in the healthy group, while the 24-hour urinary calcium level was lower than that in the healthy group. The differences were statistically significant(P<0.05). There was no statistically significant difference in urinary potassium and sodium between the two groups(P>0.05). There was no statistically significant difference in the 24-hour urinary potassium, sodium, calcium, phosphorus and magnesium levels among 166 healthy individuals by gender(P>0.05), nor was there a statistically significant difference among different age groups(P>0.05). Based on the 95% confidence interval, the normal reference value ranges of these indicators were respectively 16.70-80.70 mmol/d, 73.10-261.80 mmol/d, 1.05-8.11 mmol/d, and 5.22-27.44 mmol/d. 1.27-5.23 mmol/d. Conclusions 24-hour changes in urinary potassium, sodium, calcium and phosphorus levels are of appropriate screening value in chronic kidney disease, and their effects on the progression of chronic kidney disease can be further studied. Whether there are differences in urinary magnesium levels needs to be further confirmed by different stages of chronic kidney disease. There were no significant differences in 24-hour urine potassium, sodium, calcium, phosphorus and magnesium among the healthy people in the local area in terms of gender and age. This study successfully established the reference interval of 24-hour urine potassium, sodium, calcium, phosphorus and magnesium among the healthy people in a certain area in the central plains.
