Er:YAG激光治疗种植体周围炎疗效的meta分析
The Efficacy of Er:YAG Laser in the Treatment of Peri-Implantitis:A Meta-Analysis
【摘要】 目的 系统评价Er:YAG激光在种植体周围炎治疗中的疗效。方法 电子检索CNKI、CBM、维普、MEDLINE and PubMed等中英文数据库,收集有关Er:YAG激光治疗种植体周围炎的临床随机对照研究。采用RevMan5.3.3软件对纳入文献的牙周健康指标数据进行合并森林图分析比较。结果 最终纳入12篇文献,343名患者。meta分析结果表明,种植体周围炎患者接受Er:YAG激光治疗后3个月(Z=3.07,P=0.002)和6个月(Z=2.18,P=0.03),其菌斑指数均明显低于对照组;其探诊出血指数在治疗后3个月(Z=5.20,P<0.000 01)和6个月(Z=3.49,P=0.000 5)均低于对照组;试验组患者治疗3个月(Z=3.83,P=0.000 1)和6个月(Z=5.63,P<0.000 01)的牙周探诊深度均低于对照组;其临床附着丧失在治疗后3个月(Z=2.55,P=0.01)和6个月(Z=2.94,P=0.003)时也明显低于对照组。结论 Er:YAG激光治疗可改善种植体周围炎患者的各项牙周指标,具有较好的疗效。
【Abstract】 Objective To systematically evaluate the efficacy of Er: YAG laser in the treatment of peri-implantitis. Methods Chinese and English databases including CNKI, CBM, VIP, MEDLINE and PubMed were searched electronically to collect clinical randomized controlled trials on Er: YAG laser treatment for peri-implantitis. RevMan 5.3.3 was used to perform a meta-analysis with forest plots on periodontal health indicator data from the included studies. Results Twelve studies involving 343 patients were included. Meta-analysis results showed that the plaque index of peri-implantitis patients was significantly lower than that of the control group at 3 months(Z=3.07, P=0.002) and 6 months(Z=2.18, P=0.03) after Er: YAG laser treatment. The bleeding on probing index was lower than that of the control group at 3 months(Z=5.20, P<0.00001) and 6 months(Z=3.49, P=0.0005) after treatment. The periodontal probing depth of the experimental group was lower than that of the control group after 3 months(Z=3.83, P=0.0001) and 6 months(Z=5.63, P<0.00001). The clinical attachment loss was also significantly lower in the experimental group at 3 months(Z=2.55, P=0.01) and 6 months(Z=2.94, P=0.003) after treatment. Conclusion Er: YAG laser treatment can improve the periodontal indexes of patients with peri-implantitis, demonstrating its favorable efficacy.
