乳腺癌术后化疗患者经济毒性的潜在剖面分析及其影响因素研究

【摘要】 目的 探讨乳腺癌术后化疗患者经济毒性的潜在剖面类型及其影响因素。方法 采用便利抽样法,选取2024年10月至2025年2月,在新疆维吾尔自治区人民医院肿瘤科就诊的320例乳腺癌术后化疗患者为研究对象。采用一般资料调查表、恐动症Tampa量表(TSK)、癌症患者同伴支持量表(CaPSS)、患者报告结局的经济毒性综合评分量表进行数据收集。对结果进行潜在剖面分析并对不同类别的人口学变量差异进行检验,多元logistic回归分析患者经济毒性的影响因素。结果 本研究共纳入308例乳腺癌术后化疗患者,其经济毒性可分为“高水平经济毒性-高压力组”(31.20%);“中水平经济毒性-压力缓解组”(34.40%);“低水平经济毒性-低压力组”(34.40%)3个潜在剖面类别。疾病分期、有无转移、合并慢性疾病的数量、文化程度、家庭月收入、医疗支付方式、TSK总分、CaPSS总分在3个类别上比较,差异有统计学意义(P<0.05)。多元logistic回归分析结果显示,合并慢性疾病的数量、文化程度、家庭月收入、医疗支付方式、癌症患者同伴支持、运动恐惧是经济毒性的影响因素(P<0.05)。结论 乳腺癌术后化疗患者经济毒性存在异质性,医护人员应及时识别患者所属类别,并采取针对性的干预措施,以缓解患者经济毒性的程度。

【Abstract】 Objective To investigate the latent profiles types of financial toxicity and influencing factors in postoperative chemotherapy patients with breast cancer patients. Methods The convenience sampling method was used, a total of 320breast cancer patients who received postoperative chemotherapy at the Oncology Department of the Xinjiang Uygur Autonomous Region People’s Hospital from October 2024 to February 2025 were selected as the study subjects. Data collection was conducted using the general information questionnaire, the Tampa scale of kinesiophobia(TSK), the cancer peer support scale(Ca PSS), and the comprehensive scores for financial toxicity based on the patient-report outcome measures. Latent profile analysis was conducted on the results, and differences in demographic variables across different categories were tested. Multiple logistic regression was used to analyze the influencing factors of patients’ financial toxicity.Results A total of 308 breast cancer patients receiving postoperative chemotherapy were included. Whose financial toxicity could be divided into three latent profile groups: the“high financial toxicity-high stress group”(31.20%), the“medium financial toxicity-stress alleviation group”(34.40%), and the“low financial toxicity-low stress group”(34.40%). Disease stage, presence of metastasis, the number of concurrent chronic diseases, education level, family monthly income, medical payment method, TSK total scores, and Ca PSS total scores had statistically significant differences among the three categories(P<0.05). The results of multivariate logistic regression analysis revealed that combined with chronic diseases, educational level, family monthly income, medical payment method, peer support for cancer patients, and kinesiophobia were influencing factors of financial toxicity(P<0.05). Conclusion There is heterogeneity in financial toxicity among breast cancer patients receiving postoperative chemotherapy. Healthcare professionals should promptly identify the specific category each patient falls into and implement targeted intervention measures to alleviate the severity of their financial toxicity.
