光明工程项目下中西部5省份白内障患者术后视力恢复情况满意度、医疗机构服务满意度及影响因素

【摘要】 目的:分析光明工程项目下中西部5省份白内障患者的术后视力恢复情况满意度、医疗机构服务满意度及影响因素,为光明工程项目的优化和长期持续推进提供参考。方法:于2024年4月,选取安徽省、四川省、湖南省、广西壮族自治区、陕西省白内障患者进行问卷调查,各省份按照补助患者和非补助患者2∶1的比例共抽取8889份问卷纳入数据分析,采用描述性分析、χ~2检验、二分类logistic回归分析探讨白内障患者对术后视力恢复情况满意度、医疗机构服务满意度及影响因素。结果:在按比例抽取的8889个样本中,中西部5省份白内障患者对术后视力恢复情况“满意”占比85.95%,“不满意”占比14.05%;对医疗机构服务“满意”占比94.76%,“不满意”占比5.24%。白内障患者的年龄、患有其他相关疾病或眼病、看不清楚后到手术的时间、白内障手术次数、术后视力改善值、手术花费家庭负担程度、对医疗机构服务的满意度是白内障患者术后视力恢复情况满意度的主要影响因素(P<0.05);是否为光明工程项目补助患者、对术后视力恢复情况的满意度是白内障患者对医疗机构服务满意度的主要影响因素(P<0.05)。结论:中西部5省份白内障患者对术后视力恢复情况、医疗机构服务的总体满意度较高,补助患者满意度高于非补助患者。患者自身情况及术后视力改善值是影响术后视力恢复满意度的重要因素,术后视力恢复情况满意度与医疗机构服务满意度互为影响因素,建议扩大白内障筛查覆盖面,提高白内障手术率,加强术后随访及健康教育指导。

【Abstract】 Objective: To analyze the satisfaction with postoperative vision recovery, satisfaction with services in medical institutions, and the influencing factors among cataract patients in five provinces of the central and western regions, so as to provide references for the optimization and long-term continuous advancement of the Brightness Project. Methods: From April 2024, a questionnaire survey was conducted among cataract patients in central and western regions including Anhui province, Sichuan province, Hunan province, Guangxi Zhuang autonomous region, and Shaanxi province. A total of 8889 questionnaires were randomly selected from these provinces based on a 2∶1 ratio of subsidized to non-subsidized patients for data analysis. Descriptive analysis, chi-square tests, and binary logistic regression analysis were used to explore the satisfaction of cataract patients with postoperative vision recovery, satisfaction with services in medical institutions, and the influencing factors. Results: In a sample of 8889 individuals drawn proportionally from five provinces in the central and western regions, 85.95% of cataract patients were "satisfied" with their postoperative vision recovery, while 14.05% were "dissatisfied". For satisfaction with services in medical institutions, 94.76% were "satisfied" and 5.24% were "dissatisfied". The main factors influencing satisfaction with postoperative vision recovery among cataract patients were age, presence of other related diseases or eye conditions, time from visual impairment to surgery, number of cataract surgeries, improvement in postoperative vision, the degree of financial burden on the family due to surgery costs, and satisfaction with services in medical institutions(P<0.05). Whether the patient was a beneficiary of the Brightness Project and satisfaction with postoperative vision recovery were the main factors influencing satisfaction with services in medical institutions(P<0.05). Conclusion: Patients in the five provinces of the central and western regions have a high overall satisfaction with postoperative vision recovery and the services provided by medical institutions, with assisted patients showing higher satisfaction than non-assisted patients. The patients’ own conditions and the improvement in postoperative vision are important factors affecting satisfaction with postoperative vision recovery. Satisfaction with postoperative vision recovery and satisfaction with services in medical institutions are mutually influential factors. It is recommended to expand the coverage of cataract screening, increase the rate of cataract surgery, and strengthen postoperative follow-up and health education guidance.
