【摘要】 目的 分析布鲁顿酪氨酸激酶（BTK）抑制剂对慢性淋巴细胞白血病（CLL）患者外周血T细胞亚群和自然杀伤（NK）细胞的影响。方法 选取2020年1月至2022年6月浙江省金华市人民医院治疗的62例CLL患者作为研究对象，根据治疗方式的不同分为观察组（30例，BTK抑制剂+常规治疗）和对照组（32例，常规治疗）。比较2组患者疗效、T细胞亚群治疗前后的变化、不良反应事件情况。结果 观察组有效率83.3%，高于对照组53.1%，差异有统计学意义（P<0.05）。治疗后，观察组CD3+(1 402±478)个/μl、CD4+(700±241)个/μl、NK细胞（389±168）个/μl均高于对照组[分别为（1 052±272）个/μl、（585±192）个/μl、（302±134）个/μl]。观察组出血（1例）、消化道不适（0例）及不良反应种类数均低于对照组，差异有统计学意义（P<0.05）。结论BTK抑制剂对CLL患者具有疗效，能够促进外周血T细胞亚群和NK细胞的恢复，改善患者的免疫功能，同时减少部分不良反应，具有较高的临床应用价值。更多还原

【Abstract】 Objective To analyze the clinical study on the effects of bruton tyrosine kinase(BTK) inhibitors on peripheral blood T lymphocyte subsets and NK cells in patients with chronic lymphocytic leukemia(CLL).Methods A prospective randomized controlled trial was conducted, selecting a total of 62 CLL patients treated in the Jinhua People′ s Hospital from January 2020 to June 2022 as the research subjects. According to different treatment methods, The patients were divided into an observation group(30 cases, BTK inhibitor plus conventional treatment) and a control group(32 cases, conventional treatment). The therapeutic efficacy, changes in T cell subsets before and after treatment, and adverse reaction events were compared between the two groups. Results The effective rate of the observation group was 83.3%, higher than that of the control group at 53.1%, with a significant difference(P <0.05). After treatment, the levels of CD3+[(1 402 ±478) cells/μl vs(1 052 ±272) cells/μl], CD4+[(700±241) cells/μl vs(585±192) cells/μl], and NK [(389±168) cells/μl vs(302±134) cells/μl] in the observation group were higher than those in the control group. The observation group showed significantly lower levels of bleeding(1 case), gastrointestinal discomfort(0 cases), and the number of adverse reactions compared to the control group(P<0.05). Conclusion BTK inhibitors have significant therapeutic effects on CLL patients, promoting the recovery of peripheral blood T lymphocyte subsets and NK cells, improving patients′immune function, and reducing some adverse reactions. BTK inhibitors have high clinical application value for CLL.更多还原
