357例初治类风湿关节炎患者共病状况分析
Analysis of comorbidity in 357 patients with newly diagnosed rheumatoid arthritis
【摘要】 目的 探讨初治类风湿关节炎(RA)患者的共病状况,为临床防治提供参考。方法 选取2023年1月至2024年1月就诊于新疆维吾尔自治区人民医院风湿免疫科的357例初治RA患者,分析共病情况。结果 心脑血管疾病140例(39.22%),运动系统疾病109例(30.53%),呼吸系统疾病79例(22.13%),内分泌系统疾病66例(18.49%),血液系统疾病30例(8.40%),重叠综合征30例(8.40%),消化系统疾病13例(3.64%),传染性疾病9例(2.52%)。男性患者的冠心病、高同型半胱氨酸血症、贫血、2型糖尿病、强直性脊柱炎、痛风性关节炎占比高于女性患者(P<0.05)。≥60岁患者的冠心病、高血压、慢性阻塞性肺疾病、间质性肺病、2型糖尿病、甲亢、腰椎间盘突出、骨质疏松及乙肝占比高于<60岁患者(P<0.05)。非正常体型患者的冠心病、高血压、脑梗死、间质性肺病、干燥综合征占比高于正常体型患者(P<0.05)。不同病程患者的高血压、高同型半胱氨酸血症、慢性阻塞性肺疾病、甲亢、骨质疏松、股骨头坏死、干燥综合征、乙肝占比比较,差异具有统计学意义(P<0.05)。结论 RA共病涉及多个系统,病种多样,且受受年龄、性别、病程及体型差异影响。
【Abstract】 Objective To explore the comorbidity of patients with newly diagnosed rheumatoid arthritis(RA), and to provide reference for clinical prevention and treatment. Methods A total of 357 newly diagnosed RA patients who were treated in the rheumatology and immunology department of People’s Hospital of Xinjiang Uygur Autonomous Region from January 2023 to January 2024 were selected to analyze the comorbidities. Results There were 140 cases(39.22%) of cardiovascular and cerebrovascular diseases, 109 cases(30.53%) of motor system diseases, 79 cases(22.13%) of respiratory system diseases, 66 cases(18.49%) of endocrine system diseases, 30 cases(8.40%) of blood system diseases, 30 cases(8.40%) of overlap syndrome,13 cases(3.64%) of digestive system diseases, and 9 cases(2.52%) of infectious diseases. The proportions of coronary heart disease, hyperhomocysteinemia, anemia, type 2 diabetes, ankylosing spondylitis and gouty arthritis in male patients were higher than those in female patients(P<0.05). The proportions of coronary heart disease, hypertension, chronic obstructive pulmonary disease, interstitial lung disease, type 2 diabetes,hyperthyroidism, lumbar disc herniation, osteoporosis and hepatitis B in patients ≥60 years old were higher than those in patients<60 years old(P<0.05). The proportions of coronary heart disease, hypertension, cerebral infarction, interstitial lung disease and Sjogren’s syndrome in patients with abnormal body type were higher than those in patients with normal body type(P<0.05). There were statistically significant differences in the proportions of hypertension, hyperhomocysteinemia, chronic obstructive pulmonary disease, hyperthyroidism,osteoporosis, osteonecrosis of the femoral head, Sjogren’s syndrome and hepatitis B in patients with different course of disease(P<0.05). Conclusion RA comorbidity involves multiple systems and various diseases, and is affected by age, gender, course of disease and difference of body type.
